About Us

Longevity Health is a clinical services company and national Institutional Special Needs Plan (ISNP) founded with the purpose of optimizing the quality of life for people in long-term care by focusing on their individualized health needs.

Launched in 2018, our ISNP was designed to address the significant unmet needs of nursing home residents by establishing collaborative, value-based partnerships with long-term care facilities.

Between 2018 and 2022, we experienced rapid growth and expanded our ISNP offerings across several states, including Colorado, Florida, Illinois, Michigan, New Jersey, New York, and North Carolina. In 2023, we diversified our services by launching as an Independent Provider Association (IPA) in collaboration with leading national health insurers. This model is now available in Ohio, Pennsylvania, upstate New York, Georgia, South Carolina, Virginia, and Texas, with plans to expand into additional states.

These strategic partnerships allow us to deliver an elevated level of care to this vulnerable population. We are currently active in 13 markets and continue to see steady growth.

Our Benefits and Perks

Longevity Health offers a competitive benefit package which includes medical, dental, and vision insurance with the employee contributions to the plan being pre-tax. At this time the Company contributes contributes a larger than industry average employer contribution for employees and their covered dependents towards medical, dental and vision coverage. The Company takes great pride in offering and ultra-competitive benefit cost share options to ease the health care cost burden of our valued employees and their family members.

In addition to these pre-tax benefits, we also provide a $10,000 term life insurance policy at no cost to the employee that will be in force for the duration of their employment. We also offer the ability to enroll in voluntary term life insurance in addition to what the Company provides with the ability to enroll a spouse and dependents, as well as short and long-term disability plans. 

We also offer a HSA or FSA, a pre-tax commuter benefit program, an Employee Assistance Program (EAP), Pet insurance, Auto and Home insurance, Legal insurance, Accident insurance, Critical Illness insurance, Hospital Indemnity insurance, and a 401(k) plan with Company match and graduated vesting schedule. Through our HRIS system and benefits carriers, our employees benefit from multiple employee discount programs which are too numerous to list.

Longevity Health also provides paid continuing educations days and a stipend to pay for some or all of the continuing education costs where training, licenses, and/or certifications are a requirement of the job. In addition to the continuing education days, we provide for up to 160 hours of paid time off each year (prorated based on partial year hire date), which can carry over from one calendar year into the next, up to a maximum of 200 hours, 3 paid bereavement leave days, 5 paid jury duty days, 10 paid holidays, 2 of which are floating holidays which may be taken on days of the employees choosing.

We also offer a Company sponsored Learning Management System which, in addition to the mandatory training courses, offers an array of courses for professional growth at no cost to the employee. These courses can help to position employees for growth and advancement within the Company.
