LiveWell Choice, a subsidiary of Longevity Health, is growing and looking to hire an Advanced Practice Provider Clinical Coach - Nurse Practitioner or Physician Assistant - Corpus Christi, TX

 

General Purpose: The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Statement of Purpose, which is: To improve the health and quality of life of nursing home residents by caring for their unique medical, social, and emotional needs.

The Advanced Practice Provider Clinical Coach is a Longevity Health Advanced Practice Provider (APP – Nurse Practitioner or Physician Assistant) who is responsible for ongoing coaching and mentoring of APPs, as well as other supportive duties to clinical leadership. The APP Clinical Coach works with their assigned clinical manager(s) to ensure clinical quality, documentation and coding accuracy, and medical expense management initiatives are executed effectively. The APP Clinical Coach will flex in their role to provide clinical care as needed.

 

Key Tasks and Responsibilities:

  • In collaboration with their Clinical Manager(s), reviews daily clinical performance metrics and plans for continual improvement.
  • Continues APP orientation as the APP transitions from virtual orientation to the market team. Responsible for orientation packet completion.
  • Collaborates with the Clinical Education team to develop, review, and implement clinical education within the team.
  • Provides ongoing coaching and mentoring to market APPs.
  • Assists in collecting hospitalization data to conduct hospitalization reviews, identify root causes, and provide coaching and feedback to facilities and APPs.
  • Builds and nourishes effective nursing facility relationships to foster Longevity Health Plan program success in the market.
  • Escalates issues/concerns that may impact Longevity Health reputation, contract compliance, relationships or any state/regulatory laws/regulations or employment law.
  • Works closely with Clinical Manager(s) and Market Director determining how to improve provider quality and appropriateness of care to Longevity Health residents.
  • Identifies learning needs of team and collaborates with Clinical Manager(s) to develop a plan for improvement.
  • Performs Chart audits as per MOC guidelines for APPs and provides feedback to APPs, Clinical Manager(s), and Market Director of Clinical Services if further interventions are needed for performance improvement.
  • Collaborates with the Clinical Auditing team for continuous auditing improvement and feedback.
  • Serves as an end user tester during periods of innovation and feedback cycles up to 30% of the time.
  • Meets all APP job competencies and exhibits role model behaviors and clinical results.
  • Effectively manages assigned members and ensures all Longevity Health Plan APP duties are performed by the expected timeframes.
  • Coaches team to address and meet MTM, HEDIS, STARs metrics and compliance.
  • Assists market APPs with membership growth and during periods of high acuity by rounding and completing required visits.
  • Ensures effective communication amongst team members, builds team cohesiveness.
  • Coaches compliance with all federal and state regulatory requirements.
  • Escalates issues to clinical manager(s) as appropriate.
  • Performs other related duties as assigned.

 

Credentials & Coverage:

  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required. Will allow acute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
  • DEA License required.
  • Must have an active national provider number (NPI) and an active Medicare provider number.
  • Must be eligible to participate in the Medicare program as a provider and registered.
  • Valid state driver’s license with a good driving record and proof of automobile insurance required.
  • Auto liability insurance declaration page with minimum limits of $100,000/$300,000 for bodily injury and $100,000 for property damage with combined single limit of $300,000. May provide this proof of coverage annually. Newly onboarding employees to this job description should contact HR to discuss any adjustments required to meet this coverage.

 

Education and Training:

  • Master of Science in Nursing as Adult, Geriatric, or Family Nurse Practitioner from an accredited college or university for Nurse Practitioner candidates required.
  • Master of Physician Assistant Studies or Master of Clinical Health Services from an accredited college or university for Physician Assistance candidates required.
  • Active certification as a Nurse Practitioner or Physician Assistant from a nationally recognized certifying organization required.

 

Knowledge and Experience:

  • Minimum of three (3) years’ experience as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant).
  • One (1) year experience in a leadership role and one (1) year experience working in a Nursing Facility is strongly preferred. Will consider experience working with similar population in another setting.
  • Meets all APP job competencies and exhibits role model behaviors and clinical results.
  • Advanced care planning skills, palliative care knowledge.
  • Computer proficiency, including Microsoft Office software, ability to learn and successfully utilize electronic medical records and other clinical software. Ability to compile and analyze data to be used for management, differentiation, and marketing purposes.
  • Strong interpersonal skills are a must to effectively enhance relationships with the communities we serve, including the internal organization.
  • Able to work independently with minimal supervision getting results in a fast‐paced environment.

Founded in 2018, Longevity Health started as an Institutional Special Needs Plan (ISNP), a type of Medicare Advantage health plan that addresses significant unmet needs of long-term institutionalized members who reside in nursing homes.  Through rapid growth from 2018 to 2022, we expanded our market footprint and currently offer ISNP Medicare Advantage plans in Colorado, Florida, Illinois, Michigan, New Jersey, New York, and North Carolina.
