Longevity Health Plan is growing rapidly and is looking to add new talent to our team. Longevity Health Plan currently has operations in Colorado, Florida, Illinois, Michigan, New Jersey, New York, North Carolina, South Carolina, Georgia, Ohio, Texas, and Pennsylvania, with plans to start-up operations in other states in the very near future. Due to our rapid growth, we have an immediate need for a Human Resources Generalist. 


Company Overview:
Longevity offers an integrated health plan care management platform designed to improve clinical outcomes and reduce unnecessary medical interventions for long-term care beneficiaries living in skilled nursing facilities (SNFs). We offer Institutional Special Needs Plans (I-SNP) that are designated Medicare Advantage plans targeted to beneficiaries with Medicare part A & B who require long-term care in institutional settings. Longevity provides an advanced practice practitioner (NP or PA) supported by LHP RNs and other LHP Clinical team members to work with facility staff, physicians, and caregivers to manage member care in place, and improve their clinical outcomes. I-SNP and the Advanced Practitioner Provider/Clinical Team will improve resident and family satisfaction by providing enhanced benefits specifically tailored to the needs of facility residents. 

 

General Purpose: The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Plan Statement of Purpose, which is: To improve the health and quality of life of nursing home residents by caring for their unique medical, social and emotional needs.

The Human Resources Generalist is responsible for the local delivery and execution of core HR services to employees and managers. Performs a variety of employment related functions for the ultimate purpose of enhancing employee engagement, productivity and ensuring employee compliance with Company policies. This position is responsible for performing Human Resources-related duties on a professional level and is required to have a good working knowledge of many Human Resources functions including employment law, best practices, compensation management, and recruitment/onboarding.

 

Key Tasks and Responsibilities:

  • Partner with employees and management to communicate various human resource policies, procedures, laws, standards, and other government regulations.
  • Communicate proactively and work with departmental managers to resolve employee concerns and issues.
  • Serve as a professional and effective liaison between employees and Human Resources.
  • Implement Company culture, values, and policies.
  • Maintain and update all employee personnel records in accordance with regulatory requirements.
  • Monitor all personnel documentation expiration dates and follow up to ensure all records are in compliance.
  • Develop and implement policies and procedures and evaluate and recommend improvements to the HR processes.
  • Create and facilitate Human Resources processes; recommend changes to improve the employee experience.
  • Utilize TriNet system to maintain, organize, and update employee information.
  • Point person to coordinate new employee and open enrollment benefits enrollments.
  • Work closely with TriNet benefits contacts for enrollment, create enrollment communications plan and strategy.
  • Complete onboarding with new employees.
  • Conduct new employee orientation.
  • Delivery of the annual performance review processes, including tracking employee goals.
  • Organize and maintain employee training records and licensure requirements.
  • Create and update new Human Resources forms as needed.
  • Execute employee termination procedures.
  • Respond to unemployment benefits request.
  • Manages the exit interview process.
  • Complies with federal, state and local legal requirements by studying existing and new legislation: anticipating legislation enforcing adherence to requirements; advising management on needed actions.
  • Consistently sets the example of professionalism and standards of behavior.
  • Ability to handle sensitive information and maintain absolute confidentiality.
  • Must possess a strong sense of urgency and a strong service mentality.
  • Generate and analyze reports.
  • Performs other related duties as assigned.

 

Licensure:

  • HRCI PHR or SHRM-CP certification preferred.
  • Auto liability insurance coverage per minimum required by home state.

 

Education and Training:

  • Bachelors’ Degree in Human Resources and 2-3 years previous experience in Human Resources or, the equivalent of 10+ years’ of progressively responsible related experience.
  • Human Resources training/certification.

 

Knowledge and Experience:

  • Knowledge of labor and employment law; federal and state employment law.
  • Microsoft Office Suite experience.
  • High level of interpersonal and organization skills.
  • Ability to build rapport with all employees.
  • Strong attention to detail in composing, proofing materials, establishing priorities, and meeting deadlines.
  • Strong organizational skills.
  • Excellent written and verbal communication skills.
  • Flexibility.
  • Spreadsheet preparation and tracking skills.
  • Presentation skills.
  • Ability to prioritize and multi-task.
  • Musts be a self-starter and a team player.
