Due to our rapid growth, we have an immediate need for a Member Advocate - Music Enrichment Program Facilitator - Binghamton, New York.


Company Overview:
Longevity offers an integrated health plan care management platform designed to improve clinical outcomes and reduce unnecessary medical interventions for long-term care beneficiaries living in skilled nursing facilities (SNFs). We offer Institutional Special Needs Plans (I-SNP) that are designated Medicare Advantage plans targeted to beneficiaries with Medicare part A & B who require long-term care in institutional settings. Longevity provides an advanced practice practitioner (NP or PA) supported by LHP RNs and other LHP Clinical team members to work with facility staff, physicians, and caregivers to manage member care in place, and improve their clinical outcomes. I-SNP and the Advanced Practitioner Provider/Clinical Team will improve resident and family satisfaction by providing enhanced benefits specifically tailored to the needs of facility residents. 

 

General Purpose: The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Plan Statement of Purpose, which is: To improve the health and quality of life of nursing home residents by caring for their unique medical, social and emotional needs.

The Member Advocate-Music Enrichment Program Facilitator is a critical position to Longevity Health Plan (LHP) in building and sustaining effective relationships with LHP contracted nursing homes, members, and their loved ones.

There are two main purposes of the Member Advocate-Music Enrichment Program Facilitator (MA-MEPF) role: 1) to develop effective nursing home staff relationships, advocate for optimal member enrollment in each assigned building, offering CMS compliant resident/family events and support, to promote and strengthen facility relationships with LHP AND, 2) to successfully deploy the LHP Music Program. The MA- MEPF will establish meaningful relationships with key Nursing Home personnel, regional stakeholders, and LHP market team members, which demonstrates commitment to supporting our members, and enrollment of potential members into the Longevity Health Plan. Implements program marketing and support programs developed by the LHP team. Visits targeted NH members several times a week to check- in and visualize if the members basic needs are being met, ask if they need anything, and to escalate any concerns to designated supervisor. Develops workable relationships with NH direct care staff, social worker, and activities team. As part of these activities the MA-MEPF will schedule and deploy the LHP music program via groups or individually in the targeted nursing facilities with the targeted/eligible members for the program. The MA-MEPF is responsible for supporting on the ground deployment of music programs and clinical continuity of care support for Longevity Health Plan (LHP) members when not conducting music groups. Music support is the primary responsibility of this job working in the assigned nursing facilities and with the eligible members to successfully deploy the LHP Music Program.

When the MA-MEPF is not conducting music groups, this position will visit all LHP members in the assigned facility/facilities to catch early changes in member condition, understanding member needs, gathering member data and effectively engages current and new health plan members. The MA-MEPF engages facility staff and assists with facility and member satisfaction through effective interactions and support to LHP members. The MA-MEPF will work with the local clinical staff and leadership to assist in scheduling and deploying music groups, facility/member visits, documentation, and other health plan related tasks.

 

Key Tasks and Responsibilities:

  • Travel to targeted/assigned NHs daily: meets/greets NHA, DON, LHP APP, SW, designated LHP Program Champion, builds strong working relationships, determine if there are any issues with LHP members, and discuss how LHP can support resident/family events or other activities.
  • Works with NH designated contact to schedule music groups on the targeted NHs to be done 2-3 times a week for 45-minute groups per building to accommodate eligible LHP members. Reserves spaces in targeted NH for small groups, checks-in with facility identified contact for any special circumstances or rules around scheduling, space availability and size of groups.
  • Effectively administers music singing content to targeted residents, complete attendance logs for each group, and sends to DQCO weekly.
  • Assists with getting LHP eligible members to and from groups.
  • Attends music program training groups.
  • Identifies any issues with music groups and escalate to LHP market leaders.
  • Completes music series evaluations with LHP members and NH staff and send to LHP market leader.
  • Assists members and their loved ones in obtaining and using the Music Application.
  • Ensures group dates and times work for the targeted nursing homes. Inform LHP market leaders of any group cancellations or scheduling conflicts.
  • Preschedules groups 6 weeks at a time and confirm before leaving for the facilities, work with facilities to ensure groups start and end on time.
  • Typically, is responsible for up to 5 facilities.
  • When not conducting music groups or working with member loved ones on use of Music Application, performs duties as assigned by supervisor in the targeted/assigned building(s).
    • Visits LHP members in assigned facilities daily to determine if they need anything, to establish rapport, identify potential changes in the members’ conditions.
    • Maintains program music log with activities performed daily with members.
    • Assists with music communication to Responsible Party of members when member is stable.
  • Visits LHP members 3 times weekly in assigned buildings in between music groups to determine if they need anything, if they have any concerns, and identity any changes that should be escalated to the Market LHP leadership or NH staff.
  • Arranges family/resident events in coordination with LHP Account Managers and NH designated LHP contacts within established guidelines and budget and ensures appropriate LHP team members are in attendance.
  • Supports building support programs agreed upon by LHP Market leadership.
  • Identifies, in collaboration with NH team and LHP APP, any member related issues, and escalates to the appropriate LHP team member if unable to resolve at the nursing home level.
  • Understands and is compliant with nursing home regulations and CMS marketing rules.
  • Effectively engages members, residents, and NH teams to promote Longevity Health Plans through permission to contact generation, family meetings/resident counsels, optimizes member retention rates, member satisfaction. Engages LHP account manager and sales in all LHP promotional.
  • Performs other related duties as assigned.

 

Licensure:

  • Medical Assistant license or equivalent healthcare related certification OR Activities assistant, any therapy assistant OR an unlicensed caregiver with strong nursing home background, home health assistant background or other field-based member advocacy background but must have the ability to engage with members and be able to identify basic changes in their health status.
  • Valid state driver’s license with a good driving record and proof of automobile insurance required.
  • Auto liability insurance declaration page with minimum limits of $100,000/$300,000 for bodily injury and $100,000 for property damage with combined single limit of $300,000. You will be responsible for providing this proof of coverage on an annual basis.

 

Education and Training:

  • High School diploma/GED required.
  • Successful graduation from Medical Assistant other healthcare related training program OR related healthcare program/training preferred but not required.

 

Knowledge and Experience:

  • Nursing home background OR health plan case management OR member advocate experience preferred.
  • Positive personality, upbeat, engaging, and excellent communication skills.
  • Basic computer skills, EMR/clinical Platform/MS Office Products.
  • Willingness to actively participate and encourage participation in LHP benefits, member/resident meetings, and events.
  • Knowledge of the challenges and opportunities to serve the long-term nursing home population.
  • Must love music and be able to engage people in small groups.
  • Able to travel to 5 facilities in geography.
  • Knowledge and experience of nursing home regulations preferred.
  • Effective member and facility engagement skills.
  • Organized and effective in setting up or assisting with events, working with appropriate nursing home personnel.
  • Teamwork, and effective listening skills.
