Longevity Health Plan is growing rapidly and is looking to add new talent to our team. Longevity Health Plan currently has operations in Colorado, Florida, Illinois, Michigan New Jersey, New York, North Carolina, and Pennsylvania, with plans to start-up operations in other states in the very near future. Due to our rapid growth, we have an immediate need for a Director of Operations, Senior Living.
Company Overview:
Longevity offers an integrated health plan care management platform designed to improve clinical outcomes and reduce unnecessary medical interventions for long-term care beneficiaries living in skilled nursing facilities (SNFs). We offer Institutional Special Needs Plans (I-SNP) that are designated Medicare Advantage plans targeted to beneficiaries with Medicare part A & B who require long-term care in institutional settings. Longevity provides an advanced practice practitioner (NP or PA) supported by LHP RNs and other LHP Clinical team members to work with facility staff, physicians, and caregivers to manage member care in place, and improve their clinical outcomes. I-SNP and the Advanced Practitioner Provider/Clinical Team will improve resident and family satisfaction by providing enhanced benefits specifically tailored to the needs of facility residents.
General Purpose: The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Plan Statement of Purpose, which is: To improve the health and quality of life of nursing home residents by caring for their unique medical, social and emotional needs.
We are looking for a Director of Operations to manage and coordinate the development, launch, implementation and operations of Longevity’s new IE-SNP product. This is a fast-paced and demanding role, with an opportunity to play a key role in building a newp within a successful growth-stage company. The ideal candidate will be a strategic thinker with excellent planning, implementation and analysis skills, as well as a track record of success in managing complex, multi-faced projects.
Reporting to the Chief Development Officer, Senior Living; the Director of Operations will be responsible for supporting Longevity’s strategic growth initiatives to expand its membership to residents of Senior Living Communities, through the IE-SNP product launch and development / maintenance of contractual relationships with Post-Acute Medical Groups, Home Health and Rehabilitation and other key providers who will deliver services and who may identify potential residents for enrollment in IE-SNP or other Longevity Senior Living products. The Director of Operations will collaborate with other internal Longevity departments to assure provider payments are accurate and timely and that all contractual requirements with Senior Living Communities, Post- Acute Medical Groups, Home Health, Rehab and other key providers are met.
During Implementation and after launch, the Director of Operations will recruit, manage and train IE- SNP Member Navigators who will interface with Members / Families regarding benefits, referrals and Plan policies, as well as coordinate services with Senior Living Community staff, Post-Acute Medical Groups, Home Health, Rehab and other key providers. In addition, the Director of Operations will work closely with Longevity Implementation / Sales staff, as well as Senior Living Community designated regional staff to assure smooth enrollment and plan operations processes, in order to promote and achieve IE-SNP and other Senior Living product enrollment goals.
Key Tasks and Responsibilities:
- Develop and manage a comprehensive project plan for the IE-SNP product launch.
- Assure appropriate resource allocation to Senior Living product development, launch and operations. Coordinate and manage the work of multiple internal and external teams, including Clinical, IT, Sales and Business Development, vendor management, Compliance, and other areas.
- Develop and manage the Senior Living budget and timeline during development/implementation phases and after operational launch.
- Develop, monitor, and track short term and long-term goal performance and deliverables monthly. Identify and mitigate risks – keep the Senior Living product launch on plan and on budget.
- Set meeting agendas and objectives, and drive alignment on key decisions and next steps.
- Determine data requirements from Longevity ISNP performance which will form the basis for the IE-SNP value proposition to Senior Living Communities and key contracted providers. Collaborate with internal Longevity departments to obtain and validate data points and performance measures to be used in Senior Living Community and key provider contracts.
- In collaboration with Finance and other stakeholders develop and maintain financial models and forecasts for the IE-SNP product.
- Develop and deliver PowerPoint presentations to stakeholders on the IE-SNP product.
- Communicate regularly with stakeholders and keep them updated on the progress of the Senior Living product development / launch.
- Develop and execute a detailed IE-SNP implementation plan, in conjunction with contracted Senior Living Communities, contracted Post-Acute Medical Groups, other key providers and internal Longevity departmental leaders.
- Ensure that the IE-SNP product is launched on time and within budget, and that it meets all of the requirements of the IE-SNP business plan.
- Recruit, train and manage IE-SNP Member Care Navigators to fulfill all aspects of the IE-SNP Model of Care, all internal policies and procedures and external contract requirements of the IE-SNP product and business plan.
- Support Senior Living Business Development data and contract management needs; coordinate implementation and launch of other Longevity Senior Living products, as identified.
- Oversee implementation of the IE-SNP product with contracted Senior Living Communities, Post- Acute Medical Groups and other key provider groups.
- Performs other related duties as assigned.
Licensure:
- There are no licensure requirements for this position.
Education and Training:
- Bachelor’s degree in health care related field.
Knowledge and Experience:
- 10+ years of experience in project, program and operations management required, preferably in Medicare Advantage or Senior Living settings.
- Mastery of Microsoft Office suite including Excel and PowerPoint required.
- Familiarity with project management software and structuring large, complex projects required.
- Experience with matrixed organizations, strong analytical and problem-solving skills required.
- Excellent communication and interpersonal skills required.
- Remote position, travel as required.
