Longevity Health Plan is growing rapidly and is looking to add new talent to our team. Longevity Health Plan currently has operations in Colorado, Florida, Illinois, Michigan New Jersey, New York, North Carolina, and Pennsylvania, with plans to start-up operations in other states in the very near future. Due to our rapid growth, we have an immediate need for a Director of Partner Success.


Company Overview:
Longevity offers an integrated health plan care management platform designed to improve clinical outcomes and reduce unnecessary medical interventions for long-term care beneficiaries living in skilled nursing facilities (SNFs). We offer Institutional Special Needs Plans (I-SNP) that are designated Medicare Advantage plans targeted to beneficiaries with Medicare part A & B who require long-term care in institutional settings. Longevity provides an advanced practice practitioner (NP or PA) supported by LHP RNs and other LHP Clinical team members to work with facility staff, physicians, and caregivers to manage member care in place, and improve their clinical outcomes. I-SNP and the Advanced Practitioner Provider/Clinical Team will improve resident and family satisfaction by providing enhanced benefits specifically tailored to the needs of facility residents. 

 

General Purpose: The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Plan Statement of Purpose, which is: To improve the health and quality of life of nursing home residents by caring for their unique medical, social and emotional needs.


Job Summary:

The Partner Success Director has accountability for a key strategic payor partnership and will act as the day-to-day point person supporting all aspects of the relationship. This position will oversee all functional workstreams ensuring Longevity and our payer partner achieves our targeted growth, clinical, financial, operational, and compliance outcomes. They will lead thep, implementation, and ongoing relationship management of a new payer partner and will ensure all aspects of the relationship are on track starting from contract finalization to implementation and coordinating with Longevity and our payor partner’s functional leadership to ensure our mutual success.

A typical day-in-the-life would be to facilitate a meeting between payor client and Longevity SMEs to determine an optimal data sharing process, followed by a team meeting to determine planning priorities for the coming months. In the afternoon the Partner Success Director might work with internal team members to develop a presentation on Longevity’s performance relative to expectations, where/why gaps exist, and the specific remediation plans. In between, a client stakeholder may email/text/call the Partner Success Director with an urgent ask to confirm the ability to support a last second change in plans. At the end of day, the Partner Success Director would send a message through a workflow tool that the sales team in a partner’s facility has landed significant new membership which will help Longevity grow.


Key Tasks and Responsibilities:

  • Orchestrate the development of annual and multi-year strategic plans with our payer partner(s).
  • Support the annual new market evaluation and expansion process to enable us to rapidly grow.
  • Work with the assigned payer partner(s) to set our annual membership budget, clinical objectives, and other performance targets.
  • Understand the needs and requirements of our payer partners and our members and work with the Longevity team to design solutions, capabilities, and processes to meet these needs.
  • Manage the day-to-day work plan and resolve performance issues.
  • Create a management process with our payer partner to ensure we have alignment on our strategy and objectives, key performance indicators (KPI's)’s to measure performance, and processes to drive results.
  • Facilitate Joint Operating Committee (JOC) meetings with our payor partner to review performance against goals, address issues and plan for future expansion.
  • Create an internal management process to enable Longevity leadership to understand how we are tracking on key performance indicators, design processes for issue resolution, and drive remediation plans to ensure we hit our objectives.
  • Ensure overall compliance and delegation requirements are met.
  • Performs other related duties as assigned.


Licensure:

  • Valid state driver’s license with a good driving record and proof of automobile insurance required.


Education and Training:

  • Bachelor’s degree or equivalent work experience.


Knowledge and Experience:

  • 5-8 years of healthcare experience, preferably with health plan, post-acute and/or long-term care experience.
  • Knowledge of government sponsored insurance programs, preferably Medicare Advantage. Familiarity with Medicare Special Needs Plans (SNPs) is a bonus, but not required.
  • Value based care experience operating in an ‘at-risk’ for cost and quality environment strongly preferred.
  • Strong analytical and financial management skills.
  • Experience using healthcare data to achieve a specific objective, either in a client facing role or one where your ability to analyze and interpret information to internal stakeholders was a primary responsibility and success factor.
  • Account management and/or project management experience preferred.
  • Ability to collaborate with customers and business partners to understand their needs and requirements and then translate them to internal stakeholders via requirement documents, work plans, written, and verbal communications.
  • Organized thinker with the ability to connect the dots and ask the right questions.
  • Ability to leverage core business tools such as Excel, PowerPoint, Notion, Power BI and others to analyze and communicate data to stakeholders.
  • Organization and communication skills are paramount, demonstrated experience determining priorities and organizing a ‘to-do list’ is essential.
  • Willing to travel 10-20% of time as needed.
  • Performs other related duties as assigned.
