Longevity Health Plan is growing rapidly and is looking to add new talent to our team. Longevity Health Plan currently has operations in Colorado, Florida, Illinois, Michigan New Jersey, New York, North Carolina, and Pennsylvania, with plans to start-up operations in other states in the very near future. Due to our rapid growth, we have an immediate need for a Vice President of Quality.


Company Overview:
Longevity offers an integrated health plan care management platform designed to improve clinical outcomes and reduce unnecessary medical interventions for long-term care beneficiaries living in skilled nursing facilities (SNFs). We offer Institutional Special Needs Plans (I-SNP) that are designated Medicare Advantage plans targeted to beneficiaries with Medicare part A & B who require long-term care in institutional settings. Longevity provides an advanced practice practitioner (NP or PA) supported by LHP RNs and other LHP Clinical team members to work with facility staff, physicians, and caregivers to manage member care in place, and improve their clinical outcomes. I-SNP and the Advanced Practitioner Provider/Clinical Team will improve resident and family satisfaction by providing enhanced benefits specifically tailored to the needs of facility residents. 

 

General Purpose: The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Plan Statement of Purpose, which is: To improve the health and quality of life of nursing home residents by caring for their unique medical, social and emotional needs.

The VP, Quality is responsible for oversight and leadership of all quality and patient/member safety initiatives within the Clinical operations at LHP. This VP works closely with the Chief Clinical Officer, the Regional Clinical VPs to identify, and to implement, strategies to assure compliance with performance improvement requirements both set by the organization and by regulatory bodies. This VP will lead workgroups and projects designed to promote continuous quality improvement at LHP. This position works to eliminate preventable harm to patients/members, family members, and staff while prioritizing clinical and patient/member reported outcomes.


Key Tasks and Responsibilities:

  • Oversee the development and implementation of a comprehensive quality improvement initiatives.
  • Help to assure LHP has a culture of continuous improvement.
  • Ensure best patient/member outcomes and highest quality of care is being provided by all LHP employees.
  • Partner with Clinical Informatics and Data Analytics teams for timely, accurate and regular metrics reporting on all quality initiatives.
  • Lead the organization with direction from Senior Leadership on strategic priorities and in achieving top HEDIS, STARS ratings.
  • Develop and implement comprehensive, strategic, and tactical plans to achieve annually quality and safety goals and revise strategic plans accordingly.
  • Recommend strategic priorities to Senior Leadership rooted in organization priorities, regulations, and accreditation requirements.
  • Utilize leadership and operational expertise, knowledge and application of multiple quality improvement and patient safety frameworks, evidence-based practice, alerts, principles of high reliability, literature, and statistical processes to facilitate the operationalization of the organization's quality and safety goals.
  • Collaborate directly with Clinical Leadership and key departments to meet performance improvement requirements, both organization and regulatory requirements.
  • Oversee and coordinate the development of policies and procedures relevant to clinical quality; Ensure appropriately updated, communicated to all relevant staff, and quickly accessible.
  • Provide education to all levels of staff regarding quality metrics, trending, and their line of sight to ultimate improvement initiatives.
  • Oversee highly reliable event management processes, including a strong reporting culture, a strong event reporting system, an effective cause analysis program and a timely communication and resolution process with patients/members who experience preventable harm.
  • Assess current departmental performance on quality and safety indicators; regularly perform gap analyses and recommend focused improvement initiatives to ensure exceptional performance on those indicators.
  • Performs other related duties as assigned.


Licensure & Certification:

  • Clinical License within home state preferred.
  • Certified Professional in Health care Quality (CPHQ) preferred.
  • Valid state driver’s license with a good driving record and proof of automobile insurance required.


Education and Training:

  • Bachelor’s degree in Business Administration, Healthcare Administration, Nursing, Medicine, or a comparable area of study required.
  • Master’s degree in Business Administration, Healthcare Administration, Nursing, Medicine, or a comparable area of study preferred.


Knowledge and Experience:

  • 7+ years progressive managerial experience in healthcare and 10+ years’ progressive quality improvement experience required.
  • 3+ years Health Insurance Plan experience required.
  • Preferred experience within SNF and with Medicare Advantage Special Needs Plans preferred.
  • Knowledge of I-SNP model of care and health care delivery elements of patient/member safety, performance improvement required.
  • Comprehensive understanding of performance improvement, quality assessment, regulatory and risk management, patient/member safety, and program management.
  • Extensive knowledge base of performance improvement theory and applications; financial and data analysis, national, state, and local quality improvement initiatives regulations and accreditation requirements.
  • Experience with quality improvement tools such as PDSA, Lean, Six Sigma, robust process improvement, etc.
  • Advanced knowledge and ability to translate clinical metrics to multiple levels of clinical team members.
  • Experience in achieving results and documented track record of implementing and accomplishing quality/safety improvements across inter-disciplinary teams.
