Longevity Health Plan is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. Longevity Health Plan covers all medically necessary and preventive services covered under Medicare Part A (Hospitalization) and Part B (Medical), and Part D (Prescription Drug). We’re focused on improving our members’ physical, mental, and social well-being through personalized clinical care and therapies.

 

Longevity Health Plan is growing rapidly and is looking to add new talent to our team. Longevity Health Plan currently has operations in Florida, Illinois, New Jersey, New York, North Carolina, and Oklahoma, with plans to start-up operations in Michigan and additional states in the very near future.   Due to our rapid growth, we have an immediate need for an Implementation Operations Manager.  Ideally, this position would be located in our Corporate office in Palm Beach Gardens, FL however, remote work in another City/State is an option.

 

Position Summary

The Implementation Operations Manager is responsible for evaluating potential nursing homes for their clinical capabilities and readiness to execute, advise on findings and deliver the LHP Medicare-Advantage (MA) Institutional Special Needs Plans (ISNP) clinical model of care, and ability to work in a value-based reimbursement environment. This key role will work directly with the Local Director of Clinical Quality and Operations, Executive Director and National Clinical Implementation Senior Leader to ensure all clinical activities and requirements are ready and live to launch targeted nursing facilities for ISNP membership-based the Market Launch Plan dates. On an ongoing basis the Implementation Operations Manager will provide post-launch support as needed.  Due to an expected high volume of new building launches, the Implementation Manager will assist multiple markets, with a high level of travel required.

Essential Functions

  • Arrange facility evaluations at identified facilities. Include appropriate Facility staff, and corresponding LHP representatives
  • Evaluate how an ISNP will work within the facility
  • Identify early in the process if the facility would be best served by a Nurse Practitioner that has additional skills such as second language.
  • Report findings to team, document, and place evaluation findings and recommendations into the appropriate shared library.
  • Obtain a list of Primary Care Providers, Specialists, and Ancillary providers critical to the facility and provide to Network Lead.
  • Provide facility, credentialing contact, and Primary Care Provider contact information to clinical delivery and back-office support partners.
  • Develop a plan with facility leaders for clinical, operations and enrollment orientation and training requirements, EMR training, and to provide a workspace for Longevity Health Plan Nurse Practitioner.
  • Arrange and attend additional next-step meetings with Nursing Facility and Longevity Health Plan leadership as necessary, in conjunction with LHP team
  • Coordinate meetings with Primary Care Providers and business agent of Longevity Health Plan.
  • Facilitate facility billing meetings with Market Leader.
  • Track and manage all pre-launch and post-launch clinical activities, report on progress, issues, barriers to the local team to ensure on-time launch.
  • Schedule 7, 15, and 30-day post-launch meetings with facility leaders (DON or NHA) to ensure issues are addressed quickly gather lessons learned and best practices.
  • Assist the LHP leadership with other clinical and operational activities, whether pre or post-launch, to ensure optimal relationship building which supports continued growth and well-managed ISNP outcomes.
  • Ensure consistent adherence to Longevity Health Plan policies, procedures, compliance program, CMS requirements and any state and federal applicable requirements.
  • Performs other related duties as assigned.


Licensure

  • Clinical License or certification desired but not required.
  • License to sell Health insurance desired and will be required within 90 days of hire.

 

Education and Training:

  • Bachelor’s degree  or equivalent required, Master’s degree a plus
  • Nursing home long-term care experience desired.
  • ISNP experience desired, not required.

 

Knowledge and Experience

  • Excellent verbal and written communication skills.
  • Experience in working with managed care, working with value-based payment models in the nursing home/long-term care environment, and understand the value drivers for ISNPs.
  • Ability to travel as frequently with reasonable notice as needed.  Travel time can be up to 75%.
  • Effective, mid-level computer skills.
  • Project management experience strongly desired.
  • Ability to work as a teammate towards meeting/exceeding goals.
  • Ability to work remotely and flex work hours as needed.
