Health
Insurance: ICG provides Health Insurance from Florida Blue. Employees have the
choice of an HMO plan or a PPO plan, which allows you to seek care either
inside or outside the provider network. ICG will contribute towards the
employee’s monthly premium. Employees are eligible for health insurance
the 1st for the month after the first 90 days of employment. Eligible
employees who do not complete an enrollment at the initial time of employment
must wait until the next annual Open Enrollment period to enroll.
Dental
Plan: There are several Dental Plans/Carriers
to choose. This is a voluntary plan that provides a comprehensive list of
standard benefits to protect you against the expense of common dental
conditions and appliances. Employees have the choice of a DMO plan or a
PPO plan, which allows you to seek care either inside or outside the provider
network. This is a voluntary plan.
Short
Term Disability (STD): STD is designed to provide partial salary continuation
when an employee cannot work due to an illness, pregnancy or injury.
Coverage continues for up to 11 weeks, but can end before upon loss of
eligibility. This is a voluntary plan.
Long
Term Disability (LTD): LTD provides income replacement (a specified percentage
of a claimant’s monthly income) in the event of a covered disability.
Upon approval, benefits begin after 6 months, and will end upon loss of
eligibility. This is a voluntary plan.
Life
Insurance and Accidental Death and Dismemberment: Employees will automatically
receive $10,000 of Basic Life Insurance when enrolled in the Medical
Plan. Additional Life and AD&D Insurance may be purchased, without
having to be enrolled in the Medical Plan. AD&D provides additional coverage
for accidents that result in death, dismemberment, paralysis, loss of sight,
speech or hearing. Life and AD&D Insurance is also available for
dependents. This is a voluntary plan.
Vision
Plan: There are several Vision Plans/Carriers to choose from. All
benefits-eligible employees and their dependents are eligible to participate in
one of the two VSP Vision Plans. This is a voluntary plan.
Flexible Spending Account (FSA): Health Care and Dependent
Care Flexible Spending Accounts help reduce the employee’s taxable income by
setting aside money for medical, vision, dental or dependent care expenses on a
before-tax basis. Money is deducted from paychecks in equal
installments. The IRS will apply the “use it or lose it” rule to any money
left over in the FSA at the end of the plan year.
Workers’ Compensation: ICG insures all employees against
accidental work-related injuries under the Workers’ Compensation Act.
Regardless of the nature or severity, all injuries incurred while on the job
must be reported to an ICG official or manager immediately. Please refer

401(k) Retirement
Plan: This program is strictly voluntary, and you can join or withdraw from
the program at the beginning of any month.
Paid Time Off (PTO):
PTO is an all-purpose time-off policy for eligible employees to use
for vacation, illness or injury, and personal business. It combines traditional
vacation, sick and personal leave plans into one flexible, paid time-off
policy. Employees begin accumulating PTO on the first day of employment;
however, employees and may not use PTO during the first ninety (90) days of
employment. After that time, employees may use any earned PTO hours. Employees’ yearly PTO eligibility is based on the length of
employment as shown in the following schedule:
