Vayu Health is a non-profit healthcare community-based organization in California launching a completely different care model for low-income people living with uncontrolled chronic conditions (starting with people with diabetes) who also have high social and behavioral needs. The Opportunity

 

As we expand and transform the outpatient primary care experience, we are seeking a Director of Clinical Care leader, strategist, and change agent to join a growing (from 12 to ~40 people in the next few years) startup that is redefining outpatient whole-person care for Medicaid Managed Care rising and high-risk members through combined virtual and local in-person high touch multidisciplinary teams (i.e., community health worker, dietitian, diabetes educators, behavioral therapist, registered nurse). In this role, this person will collaboratively design/strategize, plan, and implement a value-based clinical care model and accommodate it for the growth from twelve to ~forty employees and to ~400 members at multiple sites within the next few years.

Core Responsibilities

  • Clinical Care Excellence

    • Foster a culture of innovation by creating a collaborative environment with strategic, shared decision-making in our early-stage startup. 

    • Collaboratively strategize, design, improve, and implement the care model to scale its delivery across multiple sites and care teams (CHWs, CDCES-RDs, BH practitioners, and Clinical Care Managers) while maximizing amazing equitable health, social, and well-being outcomes.

    • Develop and implement strategies to efficiently scale and execute human-centered day-to-day clinical care and operations while maintaining a high standard of patient care.

  • Clinical Team Management & Development:

    • Build, inspire, and lead high-performing, multidisciplinary care teams, CHW, BH, RD-CDCES, Clinical Care Managers, and discipline specific team leads.

    • Optimize care teams and resource allocation to ensure efficient and effective care delivery.

    • Support professional development through training, mentorship, and performance evaluation.

  • Clinical Relationships & Compliance:

    • Cultivate and maintain strong relationships with FQHCs and other provider partners.

    • Oversee credentialing processes for providers at all sites.

    • Ensure compliance with all relevant regulations, licensing requirements, and health plan contracts.

    • Collaborate effectively with internal stakeholders, including operational and administrative teams (like dyad relationships), to ensure seamless care coordination, innovation, improvement, and implementation.

    • Build and maintain relationships with licensing bodies and regulatory agencies.

    • Collaborate with care teams, especially CHWs and community partners, to identify and address healthcare needs within the community.

  • Clinical Care Operational Excellence & Risk Management:

    • Manage and optimize daily clinical operations, including troubleshooting and process improvement.

    • Develop, implement, and improve standardized procedures, protocols, workflows, and tools.

    • Leverage technology and EMR systems to enhance efficiency and improve patient outcomes.

    • Manage and improve panel sizes, schedules, and resources to maximize provider capacity and patient access as we scale.

    • Create, prioritize, and manage complex cross-collaborative portfolios and projects in small bites to meet goals and shift quickly with new insights.

    • Help implement and maintain data-driven continuous improvement initiatives to do things better and enhance patient outcomes, experience, and satisfaction.

    • Decrease clinical care risks by Identifying and developing plans, training, etc.

  • People Growth:

    • Help develop and implement a steady recruitment of new members while expanding provider networks.

    • Inspire and build an effective and confident clinical team to transform care to meet the needs of our growing number of members.

Qualifications

  • Education: Master's degree in Nursing, Public Health, or a related field.

  • Clinical background and experience in one or more areas: RN, BH, CDCES/RD, NP.

  • Minimum of 5 years of experience in a clinical leadership role, preferably in a community health or FQHC setting.

  • Proven track record of successfully leading and scaling clinical care operations.

  • Experience working in a dyad leadership model with operational/administrative partners.

  • Demonstrated ability to foster a collaborative and inclusive leadership environment.

  • Strong experience developing and implementing clinical workflows and quality improvement initiatives.

  • Experience with clinical operations management, including staffing, scheduling, and resource allocation.

  • Proficiency in using EMR systems and healthcare technology to enhance efficiency and patient care.

  • Proven project management skills with the ability to manage multiple projects and prioritize effectively.

  • Strong analytical and problem-solving skills.

  • Commitment to continuous improvement methodologies and a data-driven approach to decision-making.
