Vayu Health is an equity-focused California start-up healthcare service approach for Medicaid members living in underserved communities.

Our unique approach starts in primary care, with Medicaid Managed Care members that need extra support with their Type 1 and Type 2 diabetes. We will then expand to include more chronic conditions.

We believe critical pieces of the health care system have to fit together to make progress in human centered chronic condition care for the underserved. Our model brings together those pieces to accelerate improvement in health, equity, and costs.
