The Care Guide is responsible for delivering high-quality housing stability and care navigation services to health plan members. Serving as a trusted resource, the Care Guide conducts comprehensive assessments, develops individualized care plans, and connects members to community resources that address housing instability and related social determinants of health.

This role requires empathy, resourcefulness, and a deep understanding of the barriers faced by individuals experiencing housing instability. The Care Guide works within an interdisciplinary team to ensure coordinated, whole-person care that supports long-term member stability and well-being.

Responsibilities

  • Deliver high-quality housing stability and care navigation services to health plan members
  • Conduct comprehensive assessments to identify members’ housing and social support needs
  • Develop and implement individualized care plans tailored to each member’s situation
  • Provide guidance and support to members navigating housing instability, addressing barriers, and connecting them to appropriate community resources
  • Collaborate with interdisciplinary teams to ensure coordinated, whole-person care
  • Maintain accurate documentation and data in compliance with program standards and timelines
  • Apply expertise in housing instability and social determinants of health to inform continuous improvement of programs and services
  • Perform other related duties as assigned

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field — or equivalent professional experience
  • Demonstrated knowledge of housing instability, social determinants of health, or mental health services
  • Strong collaboration and communication skills
  • Ability to work effectively in both remote and office environments
  • Reliable internet connection (approximately 100 Mbps) to support work applications

Preferred Qualifications

  • Master’s degree in Social Work or a related field
  • Experience working with Medicaid, Medicare, or D-SNP populations
  • Bilingual in Spanish and English (written and spoken)
