About the Company

Pear Suite’s care navigation platform empowers Community Health Workers to collect, understand, and act on social determinants of health data. Pear Suite enables healthcare systems and health plans to meet the social needs of their members in a culturally sensitive, person-centered way.

Our Mission

The healthcare system lacks a standardized solution for addressing Social Determinants of Health (SDoH), a significant barrier faced by underserved populations. By empowering Community-Based Organizations to track, manage, and report on their social initiatives, we create alignment with Health Plans, enabling reimbursement and providing SDoH data to improve patient outcomes.
