Essential Functions

The Head of Community Health will develop and lead the company’s community health worker team and support population health programs across multiple healthcare sectors. This individual will be responsible for overseeing and managing the day-to-day operations of our CHW program, including recruitment, training, and supervision of CHWs, and ensuring the quality and effectiveness of the program.


Responsibilities

  • Works with the executive team to develop a comprehensive community health strategy

  • Develop and implement strategies to recruit, train, and retain a high-performing CHWs

  • Provide ongoing coaching, training, and support to CHWs to ensure they are meeting program goals and objectives

  • Ensure CHWs are providing high-quality, culturally competent, and effective care

  • Collaborate with other departments and partner companies to ensure that the CHW program is integrated with other programs and initiatives

  • Monitor and evaluate program effectiveness and provide regular reports to executive leaders, business partners, and credentialing agencies

  • Work with external partners to identify and implement best practices in community health

  • Stay up-to-date with the latest developments in CHW programs and healthcare policy, and incorporate this knowledge into program design and management

  • Participate in relevant community events and engage with community leaders to promote CHW capacity

  • Demonstrate comprehensive knowledge of relevant healthcare trends, priorities, challenges, and solutions

  • Maintain all national and state regulatory provider requirements and certifications 


Requirements

  • Master’s degree or higher in a related field (e.g., social work, nursing, community health)

  • At least 5 years of experience managing CHW teams in a healthcare setting, preferably within a health plan or health system

  • Strong leadership skills and the ability to inspire and motivate a team

  • Excellent communication skills, including the ability to communicate effectively with diverse populations and cultures

  • Knowledge of healthcare policy and regulations, as well as current trends and best practices in CHW programs

  • Experience in program design and evaluation, including the ability to use data to inform decision-making

  • Ability to develop strong professional relationships to increase partnerships and referrals

  • Strong analytical skills and the ability to solve complex problems

  • Entrepreneurial mindset, with the ability to identify and pursue new opportunities to enhance the program’s impact and effectiveness
