Reports to: Director of Operations


Essential Functions

Community Health Workers (CHWs) will work with community members, including older adults and those from diverse and underserved communities, to assess and address health and social service needs and promote well-being. Community health workers will utilize a comprehensive assessment and a range of peer coaching strategies, including health education activities, friendly check-ins, and resource linking, all by phone. Other duties may include community outreach and evaluation. 


Responsibilities

  • Participate in new team member orientation, as well as ongoing trainings

  • Clearly and efficiently follow provided processes and policies

  • Be prompt and thorough with scheduled engagements, follow-through,  documentation, and tracking progress 

  • Use your discretion and keep conversation topics appropriate

  • Converse in a friendly and engaged manner and showing empathy and discretion by building rapport and creating a positive and welcoming experience for community members

  • Have a good sense of community and available resources available to community members to access

  • Utilize community resources, social services and partnerships to assist in meeting community members needs.  

  • Able to communicate effectively by relaying questions and concerns to staff and promptly responding to team-wide communications

  • Have cultural humility, knowledge and awareness of community cultures and values


Requirements

  • Interest in serving community members of diverse backgrounds, cultures, ages, and abilities

  • Excellent written and verbal communication skills; comfortable communicating and engaging in-person or by video conference and phone with staff, community members, family members and community partners

  • Strong computer skills; ability to learn and operate web-based systems effectively (state required programs)

  • Confidentiality when dealing with confidential matters or sensitive subjects by abiding by all HIPPA guidelines

  • Highly organized with attention to detail

  • Able to maintain professionalism and flexibility in times of change and improvement

  • Flexible schedule


Minimum Qualifications

  • High school diploma or equivalent

  • Access to a working computer

  • Reliable transportation for FTF outreach 

  • No former job experience required; customer service and community healthcare experience a plus

  • English language skills required; other languages a plus
