Essential Functions

The Community Social Worker (focused on social determinants of health) will be responsible for creating social solution workflows and trainings to improve the enhance the Pear Suite platform. The Community Social Worker will work alongside product managers, engineers, and the customer success team, as well as support other strategic initiatives of the company to work towards the company’s mission and vision. 


Responsibilities

  • Create social workflows and care journeys to support the Pear Suite's platform for users and customers
  • Support workforce development by developing and deploying trainings on case management, motivational interviewing, culturally-appropriate health education, health coaching, etc.
  • Identify community-based supports to meet member needs across the social determinants of health, including transportation, food insecurity, education, housing, and employment
  • Collaborate with social service and technology partners to support resource linking
  • Serve as a key knowledge source for community health and social services


Requirements

  • Interest in serving under-resourced populations of diverse backgrounds, ages, and abilities
  • Demonstrated deep knowledge of health equity and community health
  • Demonstrated experience successfully writing and/or administering grants and proposals related to community health and health equity
  • Excellent communication skills; comfortable communicating by video conference and phone
  • Strong computer skills; ability to learn and operate web-based systems effectively
  • Confidentiality when dealing with confidential matters or sensitive subjects


Minimum Qualifications

  • Master’s degree in social work, public health, or related fields preferred
  • 3-5 years of healthcare experience, particularly with social care, and leading teams of social workers and/or community health workers
  • English language skills required
