Position Overview

We are currently looking to add a full-time Registered Nurse position to our Wade Creek Clinic team in Estacada, Oregon. The RN is involved in active patient management at our clinic, including clinical triage and patient education. Our patient population is highly complex, with an unusually high number of physical and psychiatric health issues. We are looking for someone with a high level of compassion who is patient/family-centric, familiar with health coaching principles and mutual goal setting in order to motivate patients and build engaging relationships.

Essential Functions

  • Provides evidence-based, clinically competent nursing care in a culturally sensitive manner

  • Engage and motivate patients while conducting preventative care and health promotion visits including but not limited to: tobacco cessation, diabetic foot exams, medication reviews, and Medicare AWVs. 

  • Develop Patient Chronic Disease and Preventive Health Care Plans.

  • Coordinate with outside resources, reaching out to specialty clinics when needed

  • Occasional blood draw/Phlebotomy

  • Occasional immunization/other injection clinic visits

  • Potential for occasional IV fluid management (rare)

  • Perform direct patient care, using established policies, procedures and, standards. Direct patient care visits may include:

    • Hypertension

    • Diabetes Education

    • Care Plan Reviews

    • Smoking Cessation

    • Weight Loss

    • Chronic Disease Action Plan Review (i.e. Asthma/COPD)

    • Contraceptive Education

    • End of Life Planning Discussions


Education and Experience

  • Graduate of an accredited school of nursing., Oregon RN license required

  • Minimum two (2) years of recent RN experience in acute care or ambulatory care/clinic setting required

  • Minimum of two (2) years of recent nursing experience within a psychiatric setting preferred

  • Terrific communication skills, impeccable integrity, and the ability to work well with our entire team is required

  • References are required, and a background check will be performed

  • Electronic Health Record experience is required

  • Experience in primary care population health is preferred

  • Experience in developing patient care plans is preferred
