Position Summary:

     Project Guardianship acts as court-appointed guardians for a growing population of poor, elderly, and disabled New Yorkers who have no family or friends who are able provide the support they need.  We employ a multi-disciplinary approach providing legal services, finance and asset management, property management, and care coordination for our clients. Guardianship practice generally covers a wide range of issues, including but not limited to foreclosures, public benefits, landlord/tenant law, real estate management and transactions, estate planning, medical decision-making, financial exploitation, elder abuse, and crisis management. Project staff routinely address challenging and emotionally charged issues, such as end-of-life decision-making and protecting clients from physical and financial neglect and abuse.

      As a member of the case management team the Case Manager will collaborate closely with other PG staff, various organizations and institutions to deliver effective case management services to our clients. As a case manager you will provide care coordination services and advocacy to ensure clients’ well-being, safety, and the greatest degree of independence possible, within the least restrictive environment.

Primary Responsiblities:

1. Care coordination and case management:

  • Care oversight for a caseload of 25-30 clients who are aging or living with a disability
  • Client initial intake, and monthly and annual assessments
  • Regular monitoring of client’s condition and services through monthly in-person or virtual visits, and more often via calls, if needed
  • Coordination and participation in medical and end-of-life decisions (in consultation with legal staff and medical professionals)
  • Communication and advocacy for our clients with other agencies and institutions
  • Close work and communication with clients’ families and loved ones
  • Active participation in team meetings and case reviews
  • Initiation and coordination of pre-paid funeral arrangements
  • Keeping close communication with medical and home care providers
  • Arranging client’s home cleaning and extermination, if needed


2.  Case Notes/ Reports and other documentation:

  • Keeping timely and detailed case notes, records of communications with parties
  • Timely completion of Initial, annual, and final accounting reports
  • Administrative documentation, and reports to grants/funding programs
  • Organizing/scheduling work and assignments in your job Outlook calendar
  • Updating and maintaining clients’ electronic files and records 


3. Public and private benefits, assistance programs:

  • Screening clients for government and private benefits
  • Gathering needed information and providing it to the Benefits department for applications and recertifications
  • Applying for and maintaining clients' medical insurances


    4. 
    Travelling throughout NYC (limited during pandemic) via public transportation to see clients, their families and other professionals
