We Care Jacksonville, Inc. (WeCareJax) is a non-profit 501(c)3 formed in 1993. WeCareJax seeks equitable access to quality healthcare by advancing and coordinating community-wide compassionate specialty care for the uninsured. It brings together doctors, hospitals, community clinics, and other partners to improve the health and well-being of uninsured residents of Duval County living at or below 200% of the Federal Poverty Level. WeCareJax matches volunteer specialty care providers and hospital partners with carefully pre-screened patients in need of care. With a network of more than 500 volunteer specialty physicians and clinicians and all the local hospital systems in Duval County, WeCareJax provides comprehensive specialty care for the uninsured, those experiencing homelessness, and the medically underserved of Jacksonville towards true health equity.

JOB SUMMARY

Working within the Patient Services team, provide efficient, effective, and appropriate support for referral request review and authorization; support for care plans for patients with complex conditions, including cancer; and use clinical training / experience to assist the team in its work to connect uninsured patients to compassionate specialty care and advanced diagnostics

DUTIES AND RESPONSIBILITIES

-          Triage and assess incoming referrals for specialty health care, reviewing patient information, progress notes, and referral packets for completeness before authorizing for the scheduling team  

-          Advise the Patient Services Team about protocols, conditions, and patient support as questions about care plans arise, including prioritization of referrals for patients with multiple system involvement or complex cases   

-          Initiate/maintain relationships to achieve efficient, effective results for uninsured, low-income patients in Duval County, specifically with all of WeCareJax’s free and charitable clinic referral partners and volunteer specialty providers

-          Maintain data and outcomes on patients through WeCareJax electronic medical records database and/or other tools to measure program outcomes  

-          For patients with complex conditions or with treatment plans requiring significant care coordination, serve as resource for managing transitions of care and for removing barriers to care to ensure improved health outcomes

               

MAJOR FUNCTIONS

-          Assess requests for referral from WeCareJax primary care clinic network partners, area hospitals, discharge planners and social services:

o   Evaluate requests for compassionate care for completeness, including required labs and imaging to support approval of the case by a volunteer specialty care provider or health system

o   Maintain effective and professional communication with clinic partners and their providers to ensure timely, efficient support of patient care

o   Document all steps of the process in the electronic medical record and ensure completeness of each referral for operational and quality reporting

o   Work with the Patient Services Team to support case planning and troubleshooting

-          Incorporate knowledge of mortality, morbidity, and established standards of evidence-based practice associated with a wide breadth of conditions (urgent / emergent, chronic, and acute)

-          For patients with complex conditions and care plans, serve as primary resource to patient, family members, health care providers and their support staff for questions and concerns related to the care plan and navigating transitions of care

-          Perform other duties as assigned

 

KNOWLEDGE AND SKILLS

-          Five (5) years direct nursing experience (preferred experience in a hospital, specialty provider office, or home health role)

-          Licensure/registration/certification requirements: RN unrestricted licensure in the State of Florida

-          Education: minimum - Bachelor’s Degree in Nursing

-          Self-starter with ability to function independently while supporting internal team and patient outcomes 

-          Knowledge of data entry and database utilization, specifically electronic health records; familiarity with Athena a plus

-          Competency with Microsoft Office Suite (specifically Outlook, Excel and Word)

-          Strong desire to enhance patient and community knowledge of health issues and to work with uninsured patients living at 200% or below the Federal Poverty Level, including patients who are housing insecure or experiencing homelessness

-          Bilingual strongly preferred (Spanish or Kreyòl Ayisyen)

Additional Information:

       -          Due to COVID-19, remote/home office with high-speed internet required   

-          Full-time position, with coverage 9 AM to 5 PM, Monday – Friday at a minimum; additional hours to support patient needs occasionally required
