Job Summary:

Provide health management services to individuals living with HIV/AIDS. The ideal candidate will be compassionate, creative, energetic, knowledgeable, resourceful, and reliable. The Social Wellness Advocate, Housing Specialist works within a team-oriented environment to help clients access and adhere to medical care, improve medical outcomes, and increase client self-sufficiency.

 

Essential Duties and Responsibilities:

  • Provide housing case management by working with clients to identify barriers to stable housing and develop solutions to overcome those barriers.
  • Ensure that all necessary documents are obtained as required by HOPWA & Assistance program policies and procedures as required.
  • Prepares unit sheets for work done.
  • Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
  • Ensure that all necessary documents are obtained as required by program policies and procedures at six-month and annual recertification.Generate client case notes, UOS and performance documentation (Housing plans, recertifications) to track each client’s progress monthly or as required by contract.
  • Ensure performance goals are achieved monthly and as required by contract. Examples- UOS no less than 480 per month, client contact 1x each month and updating master client tracking each month.
  • Participate in at least one outreach event monthly.

Preferred Qualifications:

  • Bachelor’s degree in social work or related field, OR 3+ years of experience.
  • Interviewing and assessment skills (strong ability to listen without judgment);

 

Cultural awareness & sensitivity;

  • Knowledge of/ability to research available community resources;
  • PC (Windows, Word, Excel, Access) and internet proficient; andExcellent written and verbal communication skills.
  • Must have knowledge in adjustment to medical diagnosis and chronic disease. Strong human relation skills are needed to interact with patients, patient’s family members, consultants, residents, medical personnel, community agencies, and other resources.
  • Experience working or volunteering in HIV/STI prevention or other community health programs preferred.
  • Experience working with electronic medical record platforms.
  • Experience conducting Motivational Interviewing (MI)Experience in solution focus interventions.
  • Experience with crisis intervention.
