Clinically, we specialize in HIV treatment and prevention, Hep C treatment and STI treatment and management. Position Overview
The Lead Case Manager will provide administrative supervision / people management for case managers in ASHwell’s Social Wellness department. They support Case Mangers to sustainably manage caseloads, provide guidance on resources, and ensure timely and accurate reporting for their team. They report to the Associate Director of Case Management and collaborate with them to develop schedules, reassign or escalate case work as needed and build a positive, engaged team culture. Their supervision measurably improves case management staff capacity. They also maintain a capped caseload and provide comprehensive social service coordination and individualized support for ASHwell’s patients and partner-referred clients. Their case management work measurably improves health outcomes through culturally competent and trauma informed case management and resource navigation. In addition to general case management, the Program Manager may specialize in working with clients who require high competency in the categories of: Expansive Gender & Sexuality, Language and Accessibility, or Crisis Support if their experience and skillset aligns. The Program Manager collaborates with an internal multidisciplinary team and external partners to ensure comprehensive care that aligns with the organization’s mission and values. They help coordinate care, provide navigation for social support services, educate on social resource management, and act as advocates in social service and public system settings. They maintain consistent and reliable communication to build a relationship of trust. The Program Manger enters, tracks and reports data regularly, oversees timely reporting from their direct reports and manages their time effectively. They collaborate with the Associate and Assistant directors to ensure team compliance with HIPPA requirements, grant deliverables, and reporting.
Core Responsibilities
People Management – 20%

Provide day-to-day supervision, support, and guidance to (up to four) case managers, ensuring consistent service delivery and adherence to program standards.

Hold weekly check in meetings with each direct report per the organization’s management practices and tools

Collaborate with the Associate Director of Case Management to grow and sustain a positive and supportive working environment

Conduct regular weekly 1:1 check-in with direct reports in adherence with the organization's management practices and tools to ensure staff care focusing on workload balance, support needs, and professional development goals.

Monitor staff performance, provide timely feedback, and address performance issues in alignment with organizational policies and HR processes.

Lead team meetings, case reviews, and workflow discussions to support coordination and problem-solving across programs

Ensure case managers’ adherence to documentation standards in compliance with HIPPA
Program & Case Management - 30%

Track and manage case load in Apricot
-
Conduct regular caseload reviews with direct reports and prepare report back for Associate Director
-
Provide 1:1 culturally competent, trauma-informed, and client-centered support
-
Collaborate closely with Peers, Rx Navigators, Intake Specialists, Heath Insurance Specialists and medical staff to ensure the most comprehensive understanding of needs, challenges and goals for each patient or client served

Conduct psychosocial and medical assessments to identify client needs, strengths, and goals

Develop individualized care plans focused on medical stability, social stability and mental health and wellness.

Establish care plans with milestones and prepare tokens and/or acts of recognition for stability and improvement throughout care plan

Coordinate and monitor care plans in collaboration with clients and their providers

Conduct outreach and follow-up to engage clients who have fallen out of care or newly diagnosed

Perform on site assessment and registration with ASHwell on Wheels (mobile medical unit) as needed via program partnerships and outreach team

Educate clients on health and resource management, treatment adherence, and their rights within the healthcare system

Use motivational interviewing and evidence-based practices to support behavior change and long-term health engagement

Support health literacy, self-advocacy, and independence among clients
-
Encourage and track partner-referred clients into ASHwell programming and services where appropriate

Maintain case load of general and specialty focus clients based on program need and referral volume

Act as point for case assignment according to specialty: Expansive Gender & Sexuality, Language and Accessibility, or Crisis Support
Administrative & Compliance – 30%

Log in daily and track notifications, messages and due dates in Apricot dashboard
-
Maintain accurate, timely, and compliant electronic records per HIPAA, organizational policy, and funding requirements via Apricot, Provide and Athena.

Participate in audits, reporting, and monitoring to ensure service quality to care coordination models (e.g., Ryan White Part B/C/D, medical case management standards).

Schedule / reschedule in-office and home visit patient-client appointments in a timely manner
-
Deliver required reports to Assistant Director in a timely manner
-
Maintain updated, verified documentation across work scope
-
Participate in and prepare for interdisciplinary case round meetings

Review and respond to voicemails daily with a consistent 24 hour or less response time
-
Ensure that all necessary documents are obtained as required by program policies and procedures at six-month and annual recertification.

Identify and request professional development opportunities related to Social Work and HIV preventative services

Required Qualifications

4+ years working experience OR bachelor's degree in social work/related field

2+ years experience serving historically marginalized communities OR lived experience
-
4+ years in any people management role

3+ years of project or program management and coordination

Prior experience conducting interviews and assessments with a nonbiased, anti-stigma and/or Harm Reduction approach
-
Ability to apply cultural awareness competency and sensitivity
-
Prior experience with or ability to research available community resources
-
Strong written and verbal communication skills
-
Prior experience offering support for processing adjustments to new medical diagnosis and/or chronic disease with patience and empathy

Strong interpersonal skills for engaging patients and clients, their family members, and broader network of supporters and community

Proficiency in Microsoft Office Suite (Teams, Word, Excel, OneDrive, PowerPoint).

Lives in Austin, TX or within the greater Austin Metro Area or within 50 miles of ASHwell headquarters at 3800 Red River

In depth understanding of gender and sexual orientation spectrums
