Performs in home visits in the San Fernando valley.

This position is part time and requires in person client visits.

Education:   Associates or Bachelors of Science degree in nursing required.     

Experience:  One year of experience required working in an acute care hospital providing care management, working with seriously ill, frail, disabled, and cognitively impaired adults.

Must hold a valid California Department of Consumers Affairs Board of Registered Nursing active license that is current and in good standing.

Hold a valid current CPR/BLS card.

Responsibilities

  • Identify the name of the patient to be reviewed.
  • Review CBAS eligibility screening tool completed  by the center prior to onsite review.
  • Request the patient’s health record.
  • Confirm that the patient, CBAS eligibility screening tool and health record all match.
  • Evaluate the potential client as a total person and identify the functional limitations which impede independent living.
  • Determine eligibility of participant for the Community Based Adult Services (CBAS) program by completing California Department of Health Care Services eligibility determination tool.
  • Assessment must be completed within 14 days of contact with Partners in Care.
  • Submit to Partners in Care for processing.

 

 

Additional Requirement:

Copy of current automobile insurance provided annually of $15,000/$30,000 for injury and $5,000 for damage to property.
