Education: Bachelor’s degree from an accredited college in Social Work, Public Health, Human Services, or a related field, or equivalent relevant experience. Master’s degree in Social Work or related field preferred.
Experience: Minimum of 3 years of experience providing or supervising Housing Navigation, Case Management, or Supportive Housing services for individuals experiencing homelessness or housing instability. Demonstrated knowledge of Los Angeles County housing, health, and social service systems required. Experience working with high-acuity populations (complex medical, behavioral health, aging, and/or justice-involved individuals) strongly preferred.
Position Summary:
The Program Manager, Community Supports – Housing oversees the implementation and performance of Medi-Cal Community Supports housing services, including Housing Navigation Services, Housing Deposits, and Housing Tenancy and Sustaining Services.
This role provides operational leadership, staff supervision, quality assurance, and compliance oversight to ensure services meet Los Angeles County Managed Care Plans’ (MCP) contractual, documentation, and audit requirements. The Program Manager supports a multidisciplinary team serving individuals experiencing homelessness, complex medical and behavioral health needs, and housing instability.
Essential Functions:
Program & Staff Management
- Supervise housing navigators through regular supervision, performance evaluation, coaching, and training.
- Ensure appropriate caseload distribution, service intensity, and staff productivity aligned with MCP contract expectations.
- Lead onboarding and ongoing training related to Community Supports service definitions and documentation standards.
Service Oversight & Quality Assurance
- Oversee outreach, intake, eligibility verification, assessment, and enrollment for Community Supports housing services.
- Ensure staff develop and implement person-centered, goal-oriented housing service plans supporting housing acquisition, retention, and health stabilization.
- Conduct routine chart reviews and corrective action planning to ensure quality, consistency, and compliance.
Documentation, Billing & Compliance
- Ensure timely, accurate, and complete documentation for all Community Supports housing services, including:
- Service notes clearly linked to the authorized Community Supports service (Housing Navigation, Housing Deposits, or Housing Tenancy & Sustaining)
- Documentation of medical or behavioral health needs, housing instability, and service necessity
- Person-centered goals, progress toward housing stability, and service outcomes
- Required participant consent, releases of information, and eligibility verification
- Monitor documentation timeliness to meet MCP billing and encounter submission deadlines.
- Ensure documentation supports audit readiness, including internal MCP reviews, DHCS monitoring, and external audits.
- Collaborate with billing, data, and compliance teams to resolve documentation deficiencies and prevent recoupments.
Housing Stability & Crisis Response
- Oversee staff response to housing-related crises involving participants, landlords, and property managers.
- Guide staff in coordinating services addressing behavioral health, substance use, benefits advocacy, and medical needs that impact housing stability.
Landlord & Community Partnerships
- Support staff in identifying rental units, engaging landlords, and facilitating lease-up processes.
- Build and maintain relationships with landlords, Housing Authorities, hospitals, Skilled Nursing Facilities, and Community-Based Organizations to support community transitions and prevent institutionalization.
- Address property management concerns, participant non-responsiveness, and tenancy risks in collaboration with partners.
Reporting & Continuous Improvement
- Review and prepare required MCP performance reports, outcome measures, and service utilization data.
- Participate in MCP meetings, case conferences, audits, and quality improvement initiatives.
- Attend trainings related to Community Supports, Permanent Supportive Housing, and best practices for high-acuity populations.
Knowledge, Skills, and Abilities:
- Strong written and verbal communication skills, including professional documentation and stakeholder collaboration.
- Ability to manage multiple priorities while maintaining accurate, compliant, and timely documentation.
- Knowledge of homelessness systems, behavioral health conditions, substance use disorders, chronic medical conditions, aging, and domestic violence.
- Familiarity with Home- and Community-Based Services, Tenant-Based and Project-Based Rental Assistance, and Permanent Supportive Housing models.
- Sound judgment in crisis situations while upholding Housing First and participant-centered approaches.
- Proficiency in Microsoft Office Suite; experience with HMIS and care management or Customer Relationship Management (CRM) systems preferred.
