The Community Supports Liaison will partner with our department Leads to support with internal and community based referrals into our various service lines with a primary focus on Private Duty.  They will work closely with all interdepartmental leads and provide referral disposition updates and update and lead member eligibility checks for referrals that enter Private Duty from external and interdepartmental pathways.

Minimum Qualifications:

B.A. in human development, social work, psychology, counseling, rehabilitation, gerontology sociology, health care administration or related field.

Detail Oriented

Experience with reconciling billable hours and invoice disputes a plus.

Experience with community referral platforms and triaging of referrals (i.e. Find Help)

Minimum of 3 years of experience working in healthcare or multidisciplinary community-based settings providing case management or care coordination services. (Adult/people with disabilities focused social work preferred.)

Minimum of three years of experience working with diverse individuals and families experiencing health related social needs.

Bilingual preferred.

Highly motivated with minimal supervision and strong attention to detail.

Excellent organizational skills and the ability to multitask and work under pressure.

Proven track record of identifying opportunities for improvement and implementing solutions proactively.

Adaptable, dependable and a team player

Excellent customer service and presentation skills.

Strong verbal and written communication skills.

Strong data entry and computer skills experience.

Proficiency in Excel, ability to filter data, create spreadsheets, and analyze data to support billing and operational functions.

Knowledge of community-based programs.

Responsibilities:

Liaison between internal departments and Network Providers, Payors, (i.e. Health System, State, Local and County personnel, etc.) to support with the triaging of inter-departmental and network referrals. referrals.

Conduct eligibility checks for inter-departmental and network referrals that enter our hub.

Conduct outreach and read script to members whom receive authorization for services.

Create new contacts and accounts in database for new providers and new member contacts.

Input referral into database and track referral progress, and disposition and assignment to network agencies.

Conduct timely follow up and conduct end of service surveys, obtain discharge and end of service documentation and enter timely notes into database.

Support with the coordination of internal and external in-service training and liaison with internal teams as appropriate to coordinate facilitation.

Attend weekly team meetings, trainings and other program related meetings that support Partners’ services and programs.

Work collaboratively with department leads to provide referral orientation material for new network members.

Manage and safeguard protected health information.
