Minimum Qualifications
•Highly motivated, responsible, and self-directed, with demonstrated organizational skills requiring minimal supervision. Enthusiastic about working in a collaborative team environment and network, to improve the lives of older adults and adults with disabilities through community wellness programs.
•Bachelor’s degree required from an accredited university in applicable fields (e.g., public health, health education, social work, and gerontology, etc.).
•Exhibits strong interpersonal/verbal and written communication skills. Experience in public speaking with strong presentation and facilitation skills.
Valid California driver’s license, reliable transportation, and proof of current liability insurance required.
Experience with Microsoft Office Suite (Word, Outlook, Teams, and Excel) preferred.
Bilingual preferred.
Responsibilities:
1.Builds and nurtures relationships with multiple community partners to introduce and embed (i.e., plan, promote, and provide) evidence-based programs (EBP) at prospective organizations throughout greater Los Angeles.
2.Working closely with leadership, colleagues, and project sites, schedules workshops and maintains EBP fidelity, practicing strong time management and organizations skills. With an attention to detail, coordinates and supports varied EBP scheduling, participant
recruitment, workshop implementation and facilitation, and data collection to meet timeline requirements and other program deliverables.
3.Leads and co-leads multiple workshops, both remotely and on-site, with senior housing communities, senior and community centers, parks and recreation facilities, faith-based organizations, and other community locations accessible to eligible participants.
4.Maintains accurate and up-to-date program databases. Maintains patient confidentiality, data safety and quality assurance, complying with HIPAA and data security guidelines.
5.Helps monitor and respond to the Community Wellness program inquiries, assisting with program referrals and workshop enrollment, with a positive customer service approach.
6.Communicates regularly and interacts positively with supervisors, team members, and community partners.
