Case Manager Position Description
Position Overview
A unique opportunity to join a team at the forefront of the growing recovery movement on the ground floor. The Case Manager is the first face anyone sees when they enter the Outpatient Center, and the last face they see when they leave. As the first impression a client gets when they come to The Haven, the Case Manager has the opportunity to engage with the students on a personal level and put them at ease during a transitory period in their lives. Beyond the interpersonal duties, the Case Manager will be responsible for helping support a client’s treatment journey through coordinating care outside of therapy, such as supportive housing, discharge planning, disability/leave paperwork, etc. Additionally, the Case Manager position involves administrative duties including, but not limited to: day-to-day management of attendance, inventory management, tidying of the office, collection of Urinalysis tests and file auditing.
Key Responsibilities
Case Management

Maintain the Haven weekly group schedule. Work with the Clinical Director on client compliance to their level of care

Create the psychiatrist’s schedule and coordinate appointments with clients

Facilitates therapeutic evidenced-based groups as needed on a substitute basis

Link clients to any community resources (Assists with GR, food stamps, TAP card)

Crisis intervention and support, when needed, case managers will assist by providing 1:1 support when a client is dysregulated, experiencing strong cravings, or in need of behavioral interventions.
Facility and Program Operations

Annotate attendance per protocol
-
Take inventory of office supplies daily and restock as needed

Keep office and front desk organized and free from clutter

Sorting and scanning of mail / correspondence as needed

Empty trash during and at the end of each shift

Creates and maintains folder inventory (intake, marketing etc)

Coordinate lunch for PHP clients
Client Relations

Helps facilitate administrative needs of Therapists as it relates to clients (e.g., copying handouts for outpatient groups)

Warmly greets all visitors to the Outpatient Center

Collects necessary information from clients following/before their appointments

Alerts proper Clinical staff in the event that a client has become dysregulated and needs a higher level of attention.

Understanding of Haven Crisis Response Plan and ability to execute as needed

Alerts Haven Care Coordinator to any irregular visitors/requests

Request Sign-ins from all non-client visitors
Urinalysis
-
Processes UAs from clients listed on daily agenda (following DHCS guidelines)

Maintains confidentiality of client Urinalysis Documentation

Logs results and completion of UAs in proper portal

Prepares UAs for mass transit via UPS on schedule as needed
Qualifications

Bachelor’s degree preferred.

Understanding the therapeutic process, fundamentals of addiction, chemical dependency, and the 12-step philosophy is strongly preferred

Ability to prioritize and perform with minimal direction

Strong interpersonal, verbal and written communication skills

Knowledge of HIPAA guidelines to protect client's confidential and sensitive healthcare information

Proficiency with Microsoft Office Suite, Google Workspace
Required Licenses/Certifications

Minimum of RAD-T certification preferred (Employer assistance with certification available)

Current Adult CPR/First Aid Certification
Confidentiality
Demonstrates knowledge and understanding of client privacy and rights; maintains
confidentiality of all financial and other sensitive materials and information in printed,
electronic or verbal form, which may jeopardize the privacy of clients and/or
employees. Accesses and uses the minimum necessary client identifiable
information to perform job responsibilities and only for authorized purposes.
Physical Requirements/Work Environments
There is some walking, bending, and kneeling; must be able to participate in moderate lifting of 30 or more pounds (packages, etc.). This position must be able to support client
weight in case of emergency situation or client disability requiring assistance in
walking, etc. There is a risk of exposure to bodily fluids infections, contagious / blood
borne diseases, and chemicals (cleaning agents), an exposed risk to highway accidents,
and an exposed risk to varying, unpredictable situations (i.e. client behavior)
