Purpose of Position

Supports the Program Manager in managing the ongoing operations of host homes.  Ensures program quality, maintains regulatory compliance, and contributes to the financial viability of the home(s).  This position is further responsible to ensure the health and safety of the clients at the site, as well as maintaining professional relationships with all Residential customers and stakeholders.

 

 

Essential Duties/Responsibilities

  1.  Supports the Program Manager in ensuring program quality of host home/companion home operations. The position is remote but coordinators are required to come into the office 2-3x per month and as needed for department meetings/trainings.
  2.  
    • Supervises host home providers/contractors, including onboarding, training, quarterly on-site monitoring of residential sites or more frequently as needed, and ongoing support.
    • Ensures implementation of clients’ Person-Centered Support Plan (Individualized Plan), attends care planning meetings and member identified team meetings, etc.
    • Coordinates and/or assessment procedures for clients.
    • Represents the department in communicating with providers, serving as a community liaison, etc.
    • Reviews all behavioral incidents, accidents, and medical emergencies, referring to the Program Manager when necessary.
    • Supports Program Manager in effectively resolving concerns and complaints.
    • Aids the Program Manager in developing and implementing site-specific efficiencies.
  1. Assists the Program Manager in maintaining regulatory compliance of Host Home/Companion Home operations.
    • Maintains current, up-to-date knowledge of all regulatory issues and changes.
    • Ensures program compliance with all standards and regulations, Imagine! policies and procedures.
    • Monitors and tracks data to ensure programmatic compliance.
  1. Assists the Program Manager with budget development and review.
  2. Ensures completion and accuracy of financial documentation.
  3. Regularly seeks to develop program-wide Quality Improvement initiatives.
  4. Fulfilling on-call responsibilities, including conflict resolution and emergency situations that meets Imagine! standards.

Other Duties/Responsibilities

  • Safeguards the health and safety of all clients at the homes.
  • Ensures presentation, including cleanliness and organization, of home(s).
  • Maintains professional relationships with Residential stakeholders.
  • Responsible for customer satisfaction for assigned home(s).

 

 

 

Job Qualifications

 

              Knowledge, Skill, and Ability:

 

  • General knowledge of the developmental disabilities system in Colorado.
  • Demonstrated skill in working with adults with developmental disabilities in a residential setting.
  • Ability to work independently and prioritize tasks/goals for self and others.
  • Effective written and verbal communication skills with individuals and groups at all professional levels.
  • Effective and creative negotiator and problem solver.
  • Effective training skills.
  • Ability to work effectively with various technology including word processing software, internet based databases, assistive technology, etc.
  • Possession of a valid driver’s license and ability to meet Imagine! driving requirements.   This position requires regular use of a personal mobile device such as a smartphone or tablet.

 

 

 Training/Education:

                            

  • Bachelor’s Degree in the Human Services field highly preferred
    OR equivalent combination of education and experience.

                                Satisfactory completion of job-specific and/or site-specific training.

 

 Experience:

  • One to three years’ experience working in the field of developmental disabilities required.
  • One year experience employed with Residential preferred.
  • Two years supervisory experience preferred.

 

 

 Working Environment/Physical Activities: 

  • Ability to lift 50 pounds.
  • Effective with shifting roles, responsibilities, and expectations in a changing environment.

       Ability to respond to emergency situations as-needed
