Salary Range:                   $20.50/hr. base pay (OT & shift premiums)  $1.75 Weekend Differential

Location:                           Longmont   

Schedule:                          Monday - Wednesday (10 hr. shift)

 

Purpose of Position

Serves as a personal advocate for a resident with developmental disabilities in the house. Coordinates and documents services received to ensure fulfillment of the resident Individual Plan. Provides for the health, safety and welfare of the residents in their homes and in the community. Implements residents’ Individual Plans. Monitors and reports on data to ensure compliance with all applicable regulating agencies. Maintains professional relationships with residents as well as members of treatment teams. Supports residents in utilizing and troubleshooting assistive technology.

 

Essential Duties/Responsibilities

1. Assists in managing the resident’s daily routine and in making his/her needs known.

  • Trains and provides support to house staff regarding daily implementation and oversight of the Primary Resident’s treatment plan, including the technology plan.
  • Monitors and reconciles personal needs ledgers weekly, while arranging unusual expenditures with Residential Site Supervisor.
  • Serves an instrumental role in planning, writing, implementing, and tracking the resident’s Individual Plan.

2. Provides for the health, safety, and welfare of the residents with disabilities in their homes and in the community.

  • Attends to resident health and safety needs according to all assessment and care plans, including medication administration, personal hygiene, and home and community safety.
  • Performs cleaning, cooking, and other site maintenance as assigned.
  • Provides transportation as required using company vehicles.
  • Assists residents and staff in utilizing and troubleshooting assistive technology, which may include monitoring resident progress.
  • Ensures the ongoing facilitation of person-centered principals in the home.
  • Deals effectively with crisis situations.

3. Implements, monitors, and reports on residents’ Individual Service and Support Plans as stated in the Individual Plan.

  • Completes all documentation and tracking timely and accurately.

4. Monitors and reports on data to ensure compliance with all applicable guidelines and regulations, including Imagine! Policies and Procedures.

  • Completes all required training in the appropriate timeframe.
  • Reports all behavioral incidents, medical emergencies, and/or significant changes in medical condition to Residential Site Supervisor, and completes all related paperwork in a timely manner.
  • Confronts conflict productively, as described in the residential training.
  • Utilizes effective time management skills.

 

Other Duties/Responsibilities

  • Strictly complies with household budget protocols and spending limits, including guidelines for resident personal needs funds.
  • Main
  • Participates regularly in technology trainings and demonstrations. Reports all technology issues in a timely manner.

 

Job Qualifications

Knowledge, Skill, and Ability:
  • General knowledge of the developmental disabilities system in Colorado.
  • Demonstrated skill in working with adults with developmental disabilities, preferably in a residential setting.
  • Effective written and verbal communication skills with individuals and groups at all professional levels.
  • Ability to productively confront conflict.
  • Demonstrated organizational and time management skills.
  • Ability to work effectively with various technology including word processing software, internet based databases, assistive technology, etc.
  • Possession of a valid driver’s license and ability to meet Imagine! driving requirements.


Training/Education:
    ·High school diploma or equivalent.     ·2 years of college preferred.     ·3 months experience at the specific Group Home/PCA preferred.     ·Ability and willingness to successfully complete required training curriculum. This includes Emergency Control Procedures, Safety Control Procedures, and IBSSP.
Experience:     · 2 years of experience preferred OR some combination of education and experience.
Working Environment/Physical Activities     ·Ability to lift 50 pounds.     ·Ability to drive a mini van and a large van with wheelchair lift.     ·Effective with shifting roles, responsibilities, and expectations in a changing environment.     ·Ability to work various shifts, including overnight shifts.    ·Willingness and ability to respond to emergency situations as they arise.
