Job Title: Bilingual Intake and Enrollment Case Manager

Department: Case Management

Compensation: $21.50/hour; Eligible for $1,500 bonus after successfully completing 90 days of employment

Reports to: Case Management Supervisor

Purpose of Position

Performs intake and eligibility determination activities for assigned group of applicants. Establishes and maintains relationships with external agencies to assist in the identification of individuals eligible for services through Imagine!. Acts as a resource to eligible individuals and families for accessing public benefits. May provide transition assistance to eligible individuals and families.  Performs all duties in compliance with applicable rules and regulations.

Essential Duties/Responsibilities

  • Performs intake and eligibility determination activities for assigned group of applicants/programs, and may assist individuals on waiting lists/not receiving services.
  • Facilitates enrollments for individuals and families as needed or requested.
  • Effectively and consistently connects eligible individuals and families with agencies and resources throughout the application process. 
  • Consults with Supervisor and other appropriate professionals as needed to reach IDD determination.
  • Establishes and maintains relationships with external agencies to assist in the identification of individuals eligible for services through Imagine!. 
  • Provides information and referrals to individuals and families throughout the application process.  May also provide crisis support as needed.
  • Ensures individuals/families that are deemed eligible for services receive the necessary information about programs and services to complete the application process and/or be placed on the appropriate waiting list(s). 
  • Assists individuals and families in resolving complaints with service providers and benefits sources.  This may include assistance with a formal appeals process.
  • Ensures all required documentation is completed in accordance with all applicable rules, regulations, policies, and procedures. 
  • May assist in contacting those on waitlist to update their status.  
  • Completes assessments to determine eligibility of individuals for long term care.
  • Provides information to individuals and families regarding service options, and may assist in acquiring needed services or supports. 
  • Maintains current knowledge base to ensure duties are performed in compliance with all applicable rules and regulations.
  • Effectively represents Imagine! in developing relationships and providing consultation to both Imagine! staff and outside agencies.  
  • This position requires regular use of a personal vehicle with liability insurance         that meets Imagine! standards.
  • This position requires regular use of a personal mobile device such as a smartphone or tablet.

Other Duties/Responsibilities

  • May provide crisis support individual and families involved in the intake and enrollment process.
  • Other duties as assigned.

Job Qualifications

Knowledge, Skill, and Ability:

·Fluency in Spanish and English. 

·Solid skills in working with people with developmental disabilities and/or their families.

·Solid independent judgment and discretion in monitoring, advising, and advocating for individuals and their families.

·Basic knowledge of family dynamics.

·Basic knowledge and understanding of behavior development.

·Ability to work independently and prioritize tasks/goals for self and others.

·Ability to consistently exercise professional discretion with providers, individuals, and families.

·Effective written and verbal communication skills with individuals and groups at all professional levels.

·Strong planning and organizational skills.

·Effective and creative negotiator and problem solver.  This includes an ability to provide crisis intervention as well as mediate disputes.

·Ability to work effectively with various technology including word processing software, internet based databases, etc.

·Receive a score of IH on the Assessment from the American council of the teaching of Foreign Languages.

·Possession of a valid driver’s license and ability to meet Imagine! driving requirements.

 

Training/Education:

Bachelor’s Degree required. Bachelor’s Degree in Human Services highly preferred.

Experience:

·Two years’ experience in a related field highly preferred.

Working Environment/Physical Activities

·Requires the ability to travel to various locations to meet with families, providers, etc.

·Effective with shifting roles, responsibilities, and expectations in a highly stressful and changing work environment. 

·Ability to respond to emergency situations as-needed.
