Case Manager

 

Department: Case Management

Reports to: Case Management Supervisor

Pay: $21.33/hr; Eligible for $1,500 bonus after successfully completing 90 days of employment

FLSA: Non-Exempt

 

Purpose of Position- Provides Case Management to an assigned case load. Develops and implements an Individualized Plan (IP). Coordinates and monitors the implementation of services/supports written in the IP. Assists clients and families in acquiring services and supports. Maintains regulatory compliance; this includes ensuring documentation is completed to maintain ongoing funding for services for clients on case load.

 

Essential Duties/Responsibilities

  • Responsible for the development and implementation of the Individualized Plan (IP).
    • Completes assessments to determine eligibility of client for long term care.
    • Leads interdisciplinary team (IDT) in annual review and development of the IP for clients. Ensures written document is completed and distributed. Using professional judgment and discretion, leads team in determining types and amounts of supports and services for clients.
    • Facilitates IDT communication throughout the year regarding IP and related services and supports.
    • Monitors services throughout the year to ensure alignment with the current client needs and the IP.
  • Ensures that the IDT reviews and revises the IP, as necessary.
  • Coordinates delivery of services and supports.
    • Counsels client and family on appropriate service options and assists in acquiring needed services/supports.
    • Utilizes professional discretion in consulting with providers to ensure best practices in service delivery.
    • Completes enrollments and terminations as needed; includes ensuring that proper notice of eligibility and discharge is provided.
    • Provides supportive counseling and crisis intervention services to clients and families as needed.
    • Ensures that all services being provided are congruent with current IP.
    • Ensures all Human Rights issues are thoroughly considered by service providers and adequately managed and fulfilled in accordance with the HRC process and recommendations.
  • Ensures all required documentation for ongoing funding and benefits is in accordance with applicable Federal, State, and Imagine! rules and regulations.
  • Completion of timely, accurate, professional case notes within established deadlines.
  • This position requires regular use of a personal mobile device such as a smartphone or tablet.

Other Duties/Responsibilities

  • Advocates for the development of services/supports needed, but not available, for people with intellectual and developmental disabilities and/or their families.
  • If position occupant has M.S.W., may supervise or assist in supervision of social work interns at Imagine!.
  • Completes any necessary follow up to investigative reports
  • Other duties as assigned.

Job Qualifications

 

Knowledge, Skill, and Ability:

    ·Solid skills in working with people with intellectual and developmental disabilities and/or their families.

·Independent judgment and discretion in monitoring, advising, and advocating for clients and their families.

·Basic knowledge of family dynamics.

·Basic knowledge and understanding of behavior development.

·Ability to work independently and prioritize tasks/goals.

·Ability to work effectively in a team setting.

·Effective and professional written and verbal communication skills with clients and groups at all professional levels.

·Strong planning and organizational skills.

·Effective and creative negotiator and problem solver. This includes an ability to provide crisis intervention as well as mediate disputes.

·Ability to work efficiently with various technology including word processing software, e-mail, databases, and general internet skills.

·Possession of a valid driver’s license and ability to meet Imagine! driving requirements.

Education:

   ·High School diploma required

   ·Bachelor’s Degree in Human Behavioral Science or related field of study highly preferred

OR

   ·Four years’ experience working in long term services and supports

OR

     ·Four years’ combination of relevant education and experience in long term services and supports.

 

Experience:

·Two years’ experience in a related field highly preferred.

 

Working Environment/Physical Activities

·Requires the ability to travel to various locations to meet with families, providers, etc.

·Effective with shifting roles, responsibilities, and expectations in a highly stressful and changing work environment.

·Ability to respond to emergency situations as-needed.
