Host Home Provider Needed – Meaningful Long‑Term Placement (Adult with Developmental Disabilities)

About the Placement

We are seeking a dedicated, patient, and compassionate Host Home Provider for this individual—an adult male with developmental disabilities who has made tremendous progress over the last 15+ years and is ready for his next residential chapter.

This is a well‑supported placement with a strong team already in place: an involved guardian and family, IMAGINE! services, behavioral supports, and a current provider who will remain available as a natural support during transition. You will not be doing this alone—this placement truly comes with a village.

About the Individual

This individual is thoughtful, funny, expressive, and full of personality. He communicates using a combination of ASL, sounds, and gestures, and understands short, direct verbal prompts very well. Building rapport and taking the time to truly listen to him is essential; he thrives when he feels heard and understood.

Things this individual loves:

  • Music (the louder, the better)
  • Dance parties, being goofy, and joking around
  • Swimming (“Hot Tub Ty”)
  • Car rides and community outings
  • Parks, hiking, movies, museums (like the Butterfly Pavilion)
  • Quiet evenings watching movies (Finding Nemo is a favorite)

He has a great appetite, is mostly independent with bathroom use (supervision required for behavioral safety), can write his ABCs, and sign his name.

Behavioral Support Needs

This individual requires Line of Sight (LOS) supervision both in the home and community due to a history of:

  • Elopement (no incidents in over 5 years)
  • Property destruction
  • Self‑injurious behavior (scratching or hitting himself when overwhelmed)

While these behaviors can sound intimidating, it is important to know:

  • Incidents are infrequent (approximately 2–3 documented behavior events per year)
  • He responds very well to proactive environmental supports.
  • His team has years of successful strategies in place that will be fully shared

Much of this individual’s success comes from thoughtful setup—minimal clutter, no glass décor, durable furnishings (an indestructible mattress will come with him), and maintaining predictable routines.

Ideal Home Environment

This placement is best suited for this individual is:

  • Calm, quiet, and uncluttered
  • Preferably without children or animals
  • Structured with consistency and clear expectations

This individual prefers being the only individual in the home or in a very small, low‑stimulus setting. He does best when changes are explained ahead of time and made with purpose.

Supports & Services

This individual is fully connected with:

  • IMAGINE! for care management, behavioral services, and day programming
  • Behavioral supports tailored to fit both this individual’s and the provider’s schedule
  • Up‑to‑date medical care (appointments primarily in Arvada/Wheat Ridge)
  • Psychotropic medication management as prescribed
