Purpose of Position- Oversees Nursing Services, including the day-to-day provision of Nursing Services to individuals enrolled in and eligible for services and support.  Works to improve coordination and communication between all members of the residential team to ensure the delivery of high quality nursing services.  Provides direct supervision to Nursing Services staff.  Supports the Assistant Director and Imagine! leaders in ensuring high-quality services, person-centered care, and compliance with internal policies, State and Federal regulations, and fiscal responsibility.  Provides some direct care and manages a client caseload.

 

Essential Duties/Responsibilities

  • Provides management-level leadership to Nursing Services. This includes the hiring, supervision and performance management of a team of Nurse Case Managers and Nursing Services staff.

 

  • Responsible for daily operations of the Nursing Services department.
    • Provides direct medical supports and assigns or delegates tasks within the scope of practice per Nurse Practice Act.
    • Assesses and evaluates health status of individuals by direct observation and evaluation, collecting objective data, analyzing data, validating data and identifying health care issues and trends.
    • Manages and participates in after-hours on-call schedules.
    • Manages nursing case management to designated client caseload.
      • Oversees nursing case reviews and individualized protocols.
      • Conducts staff meetings.
      • Monitors medication and treatment records.
    • Interfaces with physicians and other health care professionals to plan care for the client.  Processes and signs off on physician orders. 
    • Accurately transcribes medication orders as prescribed. 
    • Writes informed consent for psychotropic medications.
    • Manages infection control and respiratory protection program.
  • Supports the Assistant Director and Imagine! leadership in ensuring service quality.
    • Sets and enforces quality of care nursing services standard for individuals receiving residential services.
    • Completes periodic reports and projects, including conducting the annual planning and evaluation of the quality and quantity of services provided by Nursing Services.
    • Advises Assistant Director on current and anticipated matters, which may affect Nursing Services and/or Imagine!
    • Develops and maintains strong relationships with local and State agencies, committees, taskforces, commissions, teams, or other bodies which monitor or otherwise affect the operations of Nursing Services. Fosters alliances and coalitions with various groups.
    • Monitors for changes in regulation which may impact Imagine!’s Nursing Services.
    • Creates and monitors policies and procedures to ensure Nurse Case Manager and Direct Care Professional compliance with State and Federal regulations regarding the medical care of a designated group of individuals served by Imagine!
    • Acts as Infection Control Officer and completes annual trainings as required by regulations.

 

  • Provides after hours and weekend on-call support as needed. Support needed may be in-person, by phone, or other.

 

  • Manages Nursing Services department in accordance with all Federal, State and local rules and regulations as well as all Imagine! Policies and Procedures.

 

  • Works with the Assistant Director to develop and operate Nursing Services within the approved budget each fiscal year.

 

  • This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.

 

  • Responsible for the integrity and accuracy of data in the electronic health records and all other systems used in support of this program.

 

  • This position requires regular use of a personal mobile device such as a smartphone or tablet.

 

 

Job Qualifications

 

 

Knowledge, Skill, and Ability:

  • Solid knowledge of developmental disabilities system in Colorado.  This includes knowledge of the federal and state rules and regulations governing Nursing Services.
  • Solid knowledge of current issues and trends affecting Nursing Services.  This includes familiarity with the G-tube bill, The Nurse Practice Act (including Nurse Delegation), OSHA regulations, Medicaid, Medicare and any other pertinent legislation or regulations as required.
  • Demonstrated knowledge in diagnosis, critical assessment, medications, complications, treatment considerations and follow up for medical conditions associated with cognitive, physical and behavioral disabilities.  
  • Effective written and verbal communication skills with individuals and groups at all professional levels.
  • Effective and creative negotiator and problem solver.
  • Ability to work independently and prioritize tasks/goals for self and others.
  • Ability to work effectively with various technologies including word processing software, internet-based databases, assistive technology, etc.
  • Possession of a valid driver’s license and ability to meet Imagine! driving requirements.
  • This position requires regular use of a personal mobile device such as a smartphone or tablet.

            Training/

Education/

Licensure:

  • Bachelor’s degree in nursing / or an Associate degree RN with 5+ years clinical experience preferred, or completion of accredited Licensed Practical Nurse program.  Must maintain an active license in good standing throughout employment.
  • Specialty certification in developmental disabilities preferred.
  • Current license as a Registered Nurse or Licensed Practical Nurse in the State of Colorado in good standing.

            Experience:

  • Min 3 years as a Registered Nurse or Licensed Practical Nurse
  • Min 3 years clinical experience working in developmental disabilities, behavioral health, public clinic, or a similar community based setting preferred.
  • Min 2 years direct supervisory experience.  

            Working Environment/Physical Activities

  • Ability to lift 50 pounds.
  • Effective with shifting roles, responsibilities, and expectations in a changing environment. 
  • Willingness and ability to respond after hours and weekends as needed.
  • Willingness and ability to respond to emergency situations as they arise.
