Join the Imagine! team with this new position! The focus is to support leadership with the ongoing operations of Imagine! Residential Wrap-around Services. Works with individuals, families, and teams to access available internal and external resources that promote well-being. Ensure program quality, maintains regulatory and licensure compliance, and contributes to the financial viability of the program. This position is responsible for ensuring the health and safety of the participants and maintaining professional relationships with Imagine! participants and stakeholders.
Essential Duties/Responsibilities
Assists in ensuring program quality of Imagine! Residential Wrap-around Services.
- Plans, coordinates, and provides program activities, including scheduling of staff, vehicles, and transportation, ensuring the efficiency and quality of programming.
- Maintains a caseload of clients, including a schedule of billable activities and supports.
- Leads continuous improvement initiatives aimed at enhancing service delivery, efficiency, and client satisfaction.
- Maintains regular contact with team members, individuals, and families to provide updates, address concerns, and support service delivery. This includes, but is not limited to, monthly check-ins and quarterly meetings.
- Coordinates wrap-around services to ensure individuals meet personalized goals, ensuring alignment with Imagine!’s person-centered practices.
- Meets regularly with service leadership and attends staff meetings to stay informed of individual progress and provide individual specific training.
Promotes the health, safety, and welfare of the participants.
- Ensures health and safety requirements set forth by Imagine! guidelines as well as all regulating agencies.
- Supports individuals with skill building to accomplish their identified goals.
- Provides direct support by assisting with navigation of services, facilitating activities, and addressing immediate needs.
- Assists individuals in accessing and utilizing community-based services and service providers through coaching and mentorship.
Assists the Program Manager in maintaining regulatory and licensure compliance of the program.
- Assists the Program Manager with program compliance with all standards and regulations, including Imagine! policies and procedures.
- Monitors and tracks data to ensure programmatic compliance and the overall effectiveness and performance of the program.
- Maintains up-to-date knowledge of Imagine! services, regulatory requirements and industry best practices.
Contributes to the financial viability and integrity of the program.
- Creates and maintains program schedules, registration, and attendance.
- Provides services approved in the Person Centered Support Plan (PCSP) to clients served on a variety of HCBS waivers
- Practices fiscal responsibility through budget control as established in company policy and procedure.
- Participates in organizational and departmental efforts to improve processes and procedures.
Other Duties/Responsibilities
- Maintains and cares for Imagine! property, including supplies, vehicles, etc., being mindful of safety and cleanliness.
- Represents program in meetings and committees, as well as in promoting and marketing, as requested.
- Maintains professional relationships with Imagine! stakeholders.
- Performs other duties as assigned.
This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.
