Hiring Pay Range: $66,000 – $68,000 per year
Job Summary:
The Community Health Manager reports to the Director of Programs and supports a team of 2–3 Health Navigators in providing comprehensive health insurance navigation, including open enrollment periods (public and marketplace), SNAP & WIC assistance, health literacy, medical bill navigation, discounted care applications, payment plans, and medical assistance. They also advocate for community members facing complex medical issues, barriers to care, or needing resource connections. The Community Health Manager will audit all data and will provide training and support to ensure program outcomes and grant deliverables are met. The Community Health Manager will build a strong team dynamic, foster and maintain local community partnerships, be the liaison for FIRC at high level State and Regional meetings, and will seek out new partnership/funding opportunities.

Primary Duties:
-
Increase access to health through equitable services and programming to improve positive health outcomes
-
Support and supervise the Community Health team, perform administrative duties including scheduling and booking, timely data entry and audit, strengths-based staff supervision and accountability, schedule management, etc.

Capture, communicate and report on community health trends while advocating for change internally and externally at a community or higher level, escalating to the State, Regional, or Federal level if necessary

Participate in high level State and Regional boards, committees, forums, and meetings as the liaison for FIRC and our community and communicate back very nuanced and detailed information regarding requirements, rules, etc.

Ensure grant deliverables are being met, by auditing accuracy of data entry, holding Navigators accountable and by analyzing data and reports on a regular basis to continually improve processes

Collaborate with Finance Team, Grant Manager and Director of Programs to understand and manage complex budgets by participating in budget meetings, administering and approving Direct Emergency Funding, and submitting invoices for contractor payments

Oversee Open Enrollment (OE) periods and Special Enrollment Periods (SEP) including scheduling and coordinating events, walk-in days, partner events, marketing, outreach, communication with front desk, trainings and certifications, and all other needs to ensure a seamless OE period(s)

Understand and remain up to date on complex healthcare systems including Connect for Health Colorado, OmniSalud, Medicaid, CHP+, WIC, SNAP, etc. including bills, laws, and policies

Assist with grant writing, reporting, monitoring, auditing, evaluating and assessing client outcomes, current program trends and continuously enhancing program delivery and services

Write and update Standard Operating Procedures (SOPs) including a comprehensive Program Guide detailing appointment expectations, eligibility criteria, etc.

Responsible for hiring, training and professional development of all team members
Responsibilities include but are not limited to:

Facilitate team meetings, one-on-one check ins, walk-in events, and community events
-
Represent FIRC at various committees, taskforces and events while adhering to FIRC's mission, vision and values

Maintain strong partnerships with community organizations and statewide healthcare partners

Collaborate with program managers to enhance program delivery to best serve FIRC families and ensure highest quality of wrap around supports internally and externally are being met

Provide direct support to program participants with community health navigation and resources as needed, ensuring seamless service continuity when staff capacity is limited

Complete all required Assistor and Manager level standard trainings and remain compliant with all CAAS regulations

Always maintain HIPAA and meet confidentiality standards

General requirements of the position include:
-
Bachelor’s degree in Social Work, Public Policy, Public Health, Healthcare Administration or related field preferred; equivalent work experience in a related field will be accepted

Two years of supervisory experience required
-
GED or High School diploma required
-
Effectively demonstrate strong written and verbal communication skills in English required, and Spanish preferred
-
Previous experience and knowledge navigating healthcare systems

Knowledge of community resources in order to make appropriate referrals

Critical high-level thinking skills and confidence to speak and liaise at a high level
-
Ability to utilize technology and databases to track program outcomes

Current Driver’s License and reliable year-round transportation is required
