Founded in 2017, Covenant House Illinois (CHIL) provides critical protection and resources for youth ages 18-24 experiencing homelessness throughout Chicagoland and across Illinois. With a newly renovated base of operations, increased capacity to serve greater numbers of youth, a slate of new and expanded programs, and a recently enacted strategic vision guiding growth and service delivery over the next three years, CHIL is primed to become Chicago’s premier agency serving young people experiencing homelessness.
Primary Responsibilities:
The YDC Case Manager (CM) provides supportive services in a positive and nurturing manner to the young people we serve in Covenant House Illinois Programs using an integrated, multidisciplinary team approach. Reporting to CHILs Director of Clinical and Supportive Services, the CM of the Youth Development Coordinator will work with the Clinical team, Youth Engagement Specialists, agency staff and community partners to ensure the delivery of robust, comprehensive services to youth while ensuring implementation of agency philosophy, policy, and procedures.
Job Duties

Ensure the highest quality of delivery of services to youth in accordance with the CHIL Principles, Mission and Core Values.

Promote a safe and healing environment by utilizing the principles and practices of trauma-informed care, harm reduction and positive youth development in all aspects of work and youth engagement, including crisis prevention, intervention, and de-escalation.

Manage a regular caseload of approximately 20 YDC youth; meet with additional youth sporadically When needed, provide case management to residential youth.

Serve as a back-up for intakes in the YDC and as needed in residential spaces.

Collaborate with youth in developing both short term and long-term goals as part of developing and implementing a Success Plan.

In conjunction with the workforce team, ensure that youth have sufficient and proper ID (Birth Certificate, School Identification, State ID, and Social Security Card).

Meet with active YDC youth weekly

Enroll, exit and maintain client files in a timely manner in ETO, HMIS and other assigned databases.

Ensure that case notes are documented accurately in ETO and data entry is timely and up to date in other databases

Work with the Director of Clinical and Supportive Servies and the QI team to ensure clinical documentation is being tracked appropriate in ETO, HMIS and other program databases,

Responsible for advocating on behalf of youth with outside service providers.

Attending appointments and services when needed or requested by a youth under the guidance of the Director of Clinical and Supportive Services.

Ensure that youths files are compliant to all grant, government agencies and local, state and federal regulations.

Assist with program audit and review preparation.

Research and keep current external referral services.

Work collaboratively with both internal and external service providers.

Develop motivational tools for youth.

Maintain healthy relationships with the youth, mindful of boundaries and mutual respect.

Model caring engagement with youth.

Support Youth Engagement Specialists and other staff whenever possible and/or needed.

Participate in all required training, supervision and staff meetings.

Manage the day-to-day needs of the program i.e. processing/tracking bus cards, etc.

Help ensure that proper documentation and data is being captured in ETO.

Facilitate groups on various life-skills issues, such as hygiene, problem solving, conflict resolution, self-esteem, and problem solving 2-4 times per month.

Support Youth Engagement Specialists in providing and modeling youth engagement practices and the core values of unconditional love, absolute respect and relentless engagement on the floor

Have knowledge and experience working with the LGBTQIA+ population

Approachable, relatable, builds rapport well, and practices active listening with all individuals.

Knowledge of community social service resources to assist youth with needs related to housing, employment, education, and social-emotional well-being.

Knowledge of crisis intervention and ability to address conflict between youth; able to respond quickly and safely while calmly de-escalating crisis situations and emergencies.

Can maintain confidentiality while understanding and complying with requirements of mandated reporting, HIPAA, and other limits to confidentiality.

Willing to utilize training opportunities in support of personal and professional growth, competence development, and to increase the overall capacity to serve youth every day.

Effective verbal and written communication skills.

Proficient with standard computer operating systems.

Willingness and ability to work outside of normal business hours as well as Holidays and weekends as needed.

Able to accommodate a wide range of physical activity, including but not limited to walking long distances, moving up and down the stairs, lifting bags or boxes up to 50 pounds, driving and agency vehicle, and physically assist youth if needed.

Willingness and ability to attain a CPR/First Aid Certification.

Pass federal, state, and local background check investigations.

Organizational abilities and level of responsibility conducive to ensuring team and youth safety, keeping supplies inventoried and stocked, and tracking and reporting on grant-funded activities.

Able to articulate successes and challenges experienced by youth to inform program development and effectively communicate with CHIL staff, community partners, funding sources, and others.

Willingness to advocate for the right of all youth passionately and actively to be valued, supported, and housed.

Be willing and able to participate in a rotating on-call rotation to support program needs 24/7
-
Be willing and able to work into the evening one day each week.
Qualifications:

Master's degree in social work or related field or BA and 5+ years of youth development experience required.

2-4 years of youth development experience required.

Licensure in Social Work, psychology, counseling or human services related field (any level) preferred.

Effective written, verbal, listening and communication skills

Ability to follow instructions, prioritize assignments and work in a fast-paced environment.

Ability to exercise appropriate judgment in crisis situations, with effective interpersonal skills
-
Must be willing to function as an active member of an interdisciplinary team.

We operate a 24/7 facility which means full-time staff are expected to work some evenings, weekends and holidays.

Excellent organizational skills.
-
Ability to monitor and de-escalate a crisis situation.
