Overall Purpose of this Role: Under the supervision of the Manager, Employee Support Program provide case management to help promote financial stability among eligible team members at Central Florida healthcare facilities including connecting clients to resources and services that address the multitude of barriers that keep households from becoming financially stable.    

 

Key Accountabilities:

·        Provide direct case management services to include screening referrals for clients in need of one-time assistance; assessing clients for eligible services, and developing and implementing trainings, activities and other related services for clients as required.    

·        Coordinate the development of an individualized case plan that addresses barriers, goals, and responsibilities; monitors its implementation through participation in team meetings, interaction of team members, and direct contact with client.

·        Maintain accurate and timely records of clients through detailed case noting of assessments, contacts, and progress using appropriate case management forms (electronic and physical records). 

·        Complete referrals for services in the community and coordinates access to those services. 

·        Monitor clients’ progress in implementing a case plan, and counseling in formal and informal sessions to successfully complete the case plan.

·        Assist the Manager, Employee Support Program with data collection related to program outcomes.

·        Perform other duties as assigned.

EDUCATION AND EXPERIENCE:

·        Associate’s degree or equivalent required.  Degree in social work preferred. 

·        Case management experience preferred; prior experience with social services preferred.  

·        Prior nonprofit experience preferred.

·        Multilingual preferred.

·        Must be able to work some weekend and evening hours.     

·        Work requires excellent reading, writing and proofreading skills.  Must be able to edit, complete and assemble reports.

·        Must be able to prioritize heavy workload in fast-paced environment.

·        Must be able to maintain confidentiality.

·        Strong existing computer skills with Microsoft Office products including Word, Excel, Outlook, PowerPoint, as well as general Internet and electronic communications.

 

WORK LOCATIONS:

We are hiring multiple positions serving in healthcare facilities in the following geographic areas.  Ability to travel between sites will be necessary.

·        Downtown Orlando

·        Ocoee/Clermont/Longwood

·        St. Cloud/Dr. Phillips Area

PHYSICAL DEMANDS:

Physical demands include sitting for extended periods of time, possible bending and lifting approximately 25 pounds; regularly using the telephone and standard office equipment such as computers, copiers and fax machine, regularly moving between floors.  Reliable transportation required.  Job involves frequent and routinely intense public contact and protracted concentration.
