Manager of Clinical Programs

The Manager of Clinical Programs will be responsible for owning all clinical operations within a market, including patient engagement/experience, clinical team management, and quality. This person will report directly to the GM and will work closely with the Central Clinical Team to implement new clinical guidelines and operations and will report to the market General Manager.

Essential Functions

· Own all aspects of clinical operations within Strive Market including provider workflow, patient management and engagement, training, and quality.

· Provide management of direct patient care clinics and care management infrastructure in Strive’s market ensuring adherence to Strive clinical care model.

· Work closely with the central clinical team, the operations team, and the market General Manager.

· Monitor and maintain short and long-term goals, metrics and KPIs to drive toward those goals

· Drive continuous improvement across all aspects of clinical operations and across all markets

Minimum Qualifications

· NP, RN, or MSN with 5 years’ experience serving patients with multiple comorbid conditions or complex diseases or relevant experience.

· 5 years’ experience in outpatient care operations.

· 3 years’ experience in both telemedicine and in-clinic care delivery.

· 3 -5 years’ experience leading teams of clinicians to drive positive patient experience in multiple sites.

Preferred Qualifications:

· Nephrology, Geriatrics, Kidney Dialysis and Outpatient Case Management is strongly preferred

· Experience in a population health setting

· Ability to think critically, with demonstrated troubleshooting and problem-solving skills

· Accountable for maximizing resources and accomplishing clinical and operational key metrics

· Innovative, analytical thinker with demonstrated ability to perform root cause analysis, prepare and implement action plans, and lead improvement initiatives

· Exceptional relationship management skills and ability to influence others from C-suite to the front line

· Ability to manage multiple projects, meet deadlines, and adjust priorities appropriately in an evolving work environment with shifting time frames

· Demonstrated ability to work effectively at both detail and big-picture levels and to drive initiatives forward

· Highly detail and goal-oriented, with the ability to:

• set and track operational and clinical metrics

• ensure patient satisfaction

• ensure employee satisfaction

• client satisfaction

• cost reduction
