Care Model Training Specialist

Position Summary: 

The Strive’s Care Model Training Specialist will play an integral role in facilitating value-based care and solidifying the Care Model to onboarded market staff. Under the guidance of the Clinical Team and Head of Care Model Design, the Education specialist will assess, plan, develop, implement, facilitate, evaluate and revise educational programming for market level clinical personnel and non-nursing personnel as indicated or requested. The incumbent acts as a key resource in the orientation and continuing education of market staff. The Training Specialist implements a comprehensive education program for all incoming market team staff. Educational programs typically include, but are not limited to orientation, continuing education, case base learnings, and in-services.

 

Responsibilities : 

 

Education

1.     Collects and assesses data and information related to Strive education needs and other pertinent situations, including identification of trends, issues and supporting data to determine a comprehensive education program for market level staff.

2.     Assesses, plans, develops, implements and evaluates comprehensive Strive education programs to meet the orientation and ongoing competency needs and professional development needs of market level staff.

3.     Adheres to national education practice standards and stays abreast of and incorporates current research into education practice. Strive trainings are based on adult learning principles, organizational and learning theory consistent with Strive endorsed philosophy and direction.

4.     Provides consultation to enhance continuing education, professional development opportunities and mentorship opportunities.

5.     Develops and delivers/coordinates other educational programs as directed (e.g., case studies, in-services, continuing education) to support competence and professional growth of market level staff.

6.     Develops curricula with the Care Model and clinical team that is reflective of evidence-based practice and uses the appropriate methods of teaching to accommodate learning styles and the learning environment.

7.     Provides leadership to preceptors and serves as a role model through educational programs and ongoing coaching and mentoring.

8.     Serves as a facilitator to market level staff and actively mentors and supports their growth in the profession and clinical practice.

9.     Participates in continuing education provider unit as an Educator. Utilizes KDIGO, KDOQI, AKF, NKF, American Nurses Credentialing Center Commission on Accreditation criteria to assess needs, plan, implement and evaluate continuing nursing education activities which qualify for continuing education credit. Participates in activities to keep abreast of the changes to policies/procedures from the Clinical Team, and the expectations of the market staff.

 

 

 

Research

1.     Integrates relevant research outcomes into clinical professional development practice through effective learning activities.

2.     Evaluates outcomes of the Strive educational programs and utilizes findings for continued improvement. Incorporates objective assessment of classroom and clinical performance into evidence-based evaluation framework. Applies measurement instruments and statistical analysis, seeking consultation when needed.

3.     Supports integration of research into practice and helps develop staff members’ knowledge and skills in the research process.

 

Leadership

1.     Provides guidance, resources and knowledge for professional growth of others.

2.     Mentors colleagues as appropriate.

3.     Participates in quality performance improvement activities.

4.     Participates in ad hoc projects and educational activities as assigned by the Care Modell Team in order to advance goal achievement and patient outcomes.

5.     Demonstrates skill in navigating and communicating in a complex environment. Fosters achievement of Education/Development goals and objectives.

6.     Demonstrates responsibility for reporting to licensing, certification, accreditation, and other regulatory bodies for educational program compliance.

7.     Interacts with staff in an ethical, consistent, fair, timely, appropriate and decisive manner in accordance with organization wide and Strive policies.

8.     Identifies factors that affect staff's ability to fulfill their job responsibilities, collaborates with Clinical Team to effect appropriate change in systems.

 

 

Minimum Qualifications:  

·       Current active RN, NP, or PA state licensure.  

·       Graduate from an accredited school of nursing (Baccalaureate degree in nursing or health related field preferred).  

·       Minimum of 2 years’ experience in general nursing care or as an educator

·       Understands value-based care

·       Experience in Nephrology and working with Renal Patients preferred.  

·       Valid driver’s license and insurance when using automobile to travel for business purposes, ability to travel. Travel may be required in the future.

·       Computer knowledge preferred.  

·       Demonstrates reliability and good work habits.

Reporting To: Head of Care Model Design and Innovation

Team: Care Model Team led by Chief Medical Officer

This position involves interaction with other direct care staff, as well as support staff, physicians, and the management team.
