Founded in 2018, Strive Health is creating a new standard of kidney care to cut inpatient utilization, increase the adoption of home dialysis, and significantly improve patient outcomes and experience. Strive Health is working to reinvent how chronic kidney disease patients are engaged and treated before they reach the need for dialysis. 

Leveraging comparative and predictive data and analytics to identify patients at risk, Strive Health’s interventions significantly reduce the rate of emergent dialysis crash with pre-ESRD care and education. Strive seeks to get CKD and ESRD patients the care they need, via a comprehensive clinical model utilizing clinician extenders to engage and treat patients along their care journey.

Population Health/Clinical Strategy Lead

This individual will be responsible for supporting the Chief Medical Officer (CMO) of Strive Health on population health management and clinical strategy. Key areas of responsibility:

 

Population Health Program

Leads large scale, systemic transformational change across functional areas and geographies to drive patient centered, data-driven, team-based care that improves timely access to and provision of preventive services and chronic disease treatment. In these efforts, drives clinical and operational process improvement. Works with the CMO, market leads, quality team, and clinical operations teams to effectively achieve population health in our locations of care via the care model levers. With the care model team, develops the overall Population Health Management program and strategy. Ensures synergy of efforts across the enterprise.

·       Develops a keen understanding of Strive’s kidney care model care practice requirements for optimal, coordinated population health

·       Works with the technology team to manage patient registries and provide executive and market leadership teams the data insights required to meet the health needs of the practices across all markets

·       Synthesize, sort, format data, and generate ad-hoc reports on findings. Provides interpretations and recommendations to the leadership teams

·       Contributes to quality improvement and care redesign of population health efforts

·       Works with CMO, care model team and clinical operations to develop creative processes to proactively manage target populations

·       Assist in process mapping and development of workflows for population health management programs and care model playbooks

·       Answer and/or research questions on problems CMO and/or clinical ops have identified

·       Recognize and report data inconsistencies to appropriate personnel

Clinical Strategy

·       Position of trust and repository of ideas, strategic thought partner

·       Develop Strive’s clinical strategy alongside the CMO

·       Identify and assess risks, vulnerabilities, and opportunities for Strive’s clinical approach

·       Champion near- and long-term strategic vision for one or more clinical assets in development

·       Be a key contributor in the development of ROI analysis and build vs buy decision making in the development of clinical and technology programs and capabilities that support both core business and long-range company growth

·       Responsible for developing hypotheses, analysis framework and providing recommendations for Strive clinical initiatives

·       Supports implementation of the care model in new markets or geographies and ongoing operations

·       Works with operations, technology and business development teams to deploy care model initiatives and pilots after implementation

·       Drives process around real-time surveillance and response to risks in care strategy performance

·       Excellent analytical skills and the ability to develop and evaluate strategic business cases and financial models

·       Support the development of an analytical framework for identifying high-value opportunities within the care model

·       Works with product, data and the markets to ensure clear reporting (including metric design and dashboarding) that allows for an understanding of key Care Strategy outcomes and for rapid iteration and experimentation as needed.

·       Identifies early care strategy trends through quantitative and qualitative data, and helps prioritize impact on the care strategy work

 

 

Minimum qualifications

·       Bachelor’s degree preferred

·       5 years of related health care experience, preferably in population health, ambulatory care, community public health, case or care management, or coordinating care across multiple settings and with multiple providers.

·       Preferred 3- 5+ years of strategy consulting experience

·       Knowledge of Population Health Principles.

·       Proficient in data management and reporting

·       Strong working knowledge of Medicare/Medicaid, Commercial insurance spaces.

·       Prior use of electronic health records and other health care information systems desirable

·       Strong project management skills

·       Able to move quickly in an ever-changing environment often without supervision

·       Must demonstrate strategic thinking as well as a willingness to lead initiatives through implementation

·       Excellent communication skills including ability to persuasively and efficiently deliver presentations to senior executives

·       Ability to build strong relationships across cross-functional teams and work well with physicians, co-workers, department heads, and outside agencies.

·       Willingness to travel 10-20% of the time, depending on the portfolio of projects
