Strive Health is a next generation kidney care service provider that, in partnership with health systems and payors, drives revolutionary clinical outcomes for patients with CKD and End Stage Renal Disease patients. 

Founded in 2018, Strive Health is creating a new standard of kidney care to cut inpatient utilization, increase the adoption of home dialysis, and significantly improve patient outcomes and experience. Leveraging comparative and predictive data and analytics to identify patients at risk, Strive Health is partnering with leading health systems and payers nationwide to reinvent how primary care physicians and nephrologists identify and treat CKD patients before they have an unplanned dialysis start. 

Why become the SVP of Clinical Programs with Strive Health?
Reporting to the President & Chief Operating Officer, the Senior Vice President of Clinical Programs will be responsible for Clinical Program Management, Clinical Informatics, Platform training and performance management.  This role will also have a responsibility to drive our mission driven-culture in our markets, all while achieving the highest quality of care. This individual will collaborate closely with the Chief Medical Officer to design, implement, and deliver a new care model for kidney disease. Moreover, the Senior Vice President of Clinical Programs will partner with Strive’s clinical and operational leaders to define metrics, identify key opportunities for clinical value, and tailor care model components according to nephrology nursing policy and procedures. 

As a key leader within Strive’s growing team, s/he will collaborate cross-functionally with senior leaders and represent Strive externally with key partners, including health plans, physicians and health systems. This individual will serve as a strong cultural leader for the organization, inspiring a collaborative and mission-driven approach to care with the goal of transforming the lives of CKD and ESRD patients. S/he will lead the central Clinical Programs and Informatics team that supports and directs the clinical delivery of our programs in the markets we serve.  This individual will be joining a highly dynamic, fast-paced environment, and thus must possess and inspire a can-do mentality. S/he must be driven by a passion for value-based, integrated health care and care deeply about bringing best-in-class care models to serve CKD and ESRD patients. 

Role Responsibilities:
In conjunction with the Chief Medical Officer develop and implement Strive’s clinical programs.
Manage, monitor and improve Strive’s clinical programs. 
Provide Clinical decision support to all clinician leaders; serve as the cultural leader for the Clinical organization, inspiring a strong collaborative and mission-driven approach to care with the aim of transforming the lives of CKD and ESRD patients
Alongside the Chief Medical Officer, will work to design, implement, and deliver a new care model for kidney disease 
Partner with Strive clinical and operational leaders to define success metrics, identify key opportunities for clinical value, and tailor care model components according to nephrology nursing policy and procedures
Represent Strive externally with health system and health plan partners to reinforce Strive’s care model; lead in the development of future high-value relationships 
Collaborate cross-functionally with senior leaders to set the strategy and be a true thought partner in carrying out Strive’s mission 

Role Qualifications:
Has held direct oversight of large distributed care teams mainly comprised of Nurses Practitioners and/or Registered Nurses; possesses an intrinsic ability to form relationships with and inspire clinicians and non-clinicians
Has experience with Population Health and Value Based Care principles.
Has deep knowledge of Care Management and Utilization Management practices.
Has established operational processes to meet the demands of growth in a rapidly scaling clinical care setting
Experience with co-occurring chronic medical and behavioral health conditions, implementing solutions to more effectively treat the most vulnerable populations
Possesses a can-do mentality; excited about the opportunity to “roll their sleeves up” and change the healthcare system
Demonstrated track record of building and mentoring highly collaborative care teams; can attract, develop, and retain A-level talent
Possesses a deep understanding of challenges and opportunities in kidney care space, including complexities of CKD and ESRD patient cohorts, regulatory and reimbursement environment, industry and market dynamics; Nephrology nurse by training ideal
