The dietician’s role at the facility is to ensure quality nutritional care of patients who have chronic renal failure. Adherence to all policies and procedures is required.
Bachelor’s degree or advanced degree from an accredited college or university with a major field of interest in food and nutrition or dietetics. Must be a registered dietitian with the Commission on Dietetic Registration.
A minimum of one year of professional work experience in clinical nutrition after registration as a dietitian.
Summary of Duties:
Provide ongoing monitoring of subjective and objective data to determine the need for timely intervention and follow-up. Measurement criteria include but are not limited to weight changes, blood chemistries, adequacy of dialysis, and medication that affects nutrition status and potentially causes adverse nutrient interactions.
Recommend therapeutic diets in consideration of cultural preferences and changes in treatment based on the patient’s nutritional needs in consultation with the patient’s physician.
Counsel a patient, a patient’s family, and a patient’s significant other on prescribed diets and monitoring adherence and response to diet therapy.
Refer a patient for assistance with nutrition resources, such as financial assistance, community resources, or in-home assistance.
Follow federal and state regulations.
Collect objective and subjective patient data to assess nutrition status within two weeks or seven treatments from admission to the facility, whichever occurs later. A patient comprehensive nutrition assessment with an educational component shall be completed within 30 days or 13 treatments from admission to the facility, whichever occurs later.
Conduct a nutrition reassessment annually or more often if indicated.
Participate in monthly clinics with the patient and family members to review the prescribed diet and dietary compliance.
Be available to answer questions and discuss problems with dialysis patients when excessive weight gain occurs or when other instances of suspected dietary non-compliance occurs.
Design educational materials concerning diet for the dialysis patient, conduct seminars and classes for dialysis patients concerning aspects of their diets, and is available for diet education to outpatients who are not yet ready for dialysis.
Be available for staff education regarding the nutritional care for the patient.
Participate in monthly interdisciplinary patient care conferences.
Help maintain a productive quality assurance program and participate in the unit’s quality assurance activities under the direction of the Quality Assurance Committee.
Attend monthly Quality Assurance Performance Improvement (QAPI) meetings
Ensure that follow-up progress notes are written in each patient’s records each month and conduct projects under the supervision of the medical director relating to diet, renal disease, and enhancement of nutritional care of chronic patients.
Perform regularly scheduled evaluations of standard menus for renal patients, revising and/or updating menus as necessary, and participate in monthly interdisciplinary patient care conference.
Document the patient’s care plan and long-term program on a monthly basis or as needed. The dietician’s participation in Therapeutic Team Meetings will provide other team members with important nutritional information regarding each patient and his/her plan of care.
Meet with salespeople concerning products that could be used in providing nutritional care for renal patients and make recommendations for possible purchases.
Be available at the facility during scheduled treatment times. Patient access to services may require an appointment.
Ensure one full-time equivalent of dietician time shall be available for up to 100 patients with the maximum patient load per full-time equivalent of dietitian time of 125 patients.
Write out diets and confer with the patient physician.

Because of possible contact with infectious diseases, good safety and personal hygiene habits are important. Because of potential exposure to environmental hazards, caution must be exercised to prevent injury. Injuries could be serious.
