Strive’s Specialized Clinical Team works with the patient’s Nephrologist and Primary Care Physician and adds additional resources to the healthcare system to massively improve outcomes. This team is led by a Medical Director and is comprised of Nurse Practitioners, RN Case Management, Care Coordinators, Social Worker, Dietician, Pharmacist and Educators.
We deliver a turn-key kidney care service line that will cut inpatient utilization [by 50%[, increase [triple] rates of home dialysis adoption, and significantly improve patient outcomes and experience.

Purpose & Scope
As part of Strive’s interdisciplinary clinical care team, the NP will utilize their advanced education, skills, and judgment to deliver advanced nursing care for CKD and ESRD patients, ensure the safe and effective delivery of patient care, and achieve optimal patient outcomes. The NP will achieve these goals using critical analysis and problem-solving skills and through evidence-based decision making.
Principle Duties & Responsibilities
§ Will be accountable for a panel of CKD and ESRD patients
§ Utilizes advanced education, skills and judgment to appropriately assess patient needs, deliver advanced quality nursing care, and achieve optimal patient outcomes for patients with chronic kidney disease and ESRD.
§ Obtains patient history, performs appropriate physical exams, orders and interprets diagnostic tests and lab values to identify and assess patient’s health care needs.
§ On an ongoing basis, monitors and documents patient condition, response to the care plan and significant incidents. Formulates a plan and prognosis based on patient’s condition.
§ Alongside with other members of the interdisciplinary team, will assure that a patient’s need for immediate and/or emergent care is handled promptly and appropriately.
§ Follows established best practices for kidney care as detailed in Strive’s Clinical Practice Guidelines for Nephrology Care.
§ Excels at developing strong patient/family relationships that fosters engagement and best outcomes for all aspects of Strive Health’s Model of Care – e.g. from basic customer service to empowerment and support when a patient is at high risk or struggling or making a tough decision.
§ Fosters a relationship with patients across their healthcare journey - with a foundation of respect, honesty, empathy, no judgement and follow-through – that promotes patient’s self-care and empowerment, while being an advocate, navigator, trusted clinician, and safety net for the patient.
§ Skilled and passionate caring for patients with complex needs and is excited about the opportunity to increase the scope of care under the Strive Model of Care.
§ Collaborates well with all levels of a clinical team (from Medical Assistants to Physicians), and, when appropriate, is able to align and influence the care needs of a patient.
§ Serves as a liaison with dialysis facilities, providing care plan updates to help streamline the transitions in care
§ Looks for and takes advantage of opportunities to improve skills (e.g. clinical, team-based, other soft skills).
§ Remains up-to-date and current on advances and trends in renal care treatment, guidelines, policies and procedures.
§ Graduate of accredited School of Nursing Nurse Practitioner Program
§ Minimum of three (3) years' experience as a Nurse Practitioner.
§ Current state specific licensure required.
§ Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
§ Preferred experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or experience in internal medicine, cardiology, Nephrology or Case Management.
§ Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required.
