    Position Summary

    In the position of the RN Chronic Care Coordinator, Remote will perform telephonic encounters with patients on behalf of our partners and develops detailed care plans. This is an Independent Contractor position and does not offer employee benefits. 

    Wellbox is seeking Registered Nurses to work full-time from their home office while complying with HIPAA privacy laws. You will set your own hours and will not be held to a daily work hour schedule. Wellbox wants its team members to have the flexibility to balance their work-life with their home life. Full-time team members will typically need to dedicate an average of 40 hours per week to care for their assigned patients. This unique business model allows you to choose what days and what hours of the day you dedicate to caring for your patients.

    Essential Functions

    •       Manage and support our patient’s health care needs via phone conversations

    •       Use various technology platforms and EHRs to conduct and document visits

    •       Manage and support a personalized care plan for each patient to address their physical, mental, and preventative health

    •       Coach patients to reach their goals of medication management, following medical treatment plans, healthy eating, weight management, increasing activity, stress reduction, sleep improvement, etc.

    •       Connect patients with professionals such as nutritionists, nurses, and doctors who can also assist them in meeting their health care needs

    •       Prepare patients for appointments and procedures. Provide follow-up as needed

    •       Coordinate some administrative aspects of medical record recovery, appointment management, and insurance claims

    •       Review insurance plans and suggest tips to get the most from their benefits

    •       Proactively suggest services or preventive screenings relevant to each patient

    Work Environment

    This job operates in the comfort of your home setting and routinely uses standard office equipment such as computers and headsets for internet calls. You will be provided with all equipment and software needed to perform your duties. You will need to have wired, high-speed internet, a private space free from noise, and an ergonomic workspace for your computer.

    Physical Demands

    This is a sedentary position, but some equipment setup is required. 

    Position Type and Expected Hours of Work

    This is an independent contractor position. Nurses are expected to set and manage their own schedules. Patient calls may be made between 8 am-8 pm in the patient’s time zone. Wellbox is looking for team members that can complete 200-350 patient encounters monthly. This would require approx. 32-40 hours weekly.

    Pay Structure

    Wellbox utilizes a productivity-based pay structure and pays $9.50-$12.50 per completed patient encounter. A patient encounter will take a minimum of 20 minutes (time is cumulative). 

    Required Education and Experience

    •       Active RN License

    •       Specialty certification preferred

    •       A minimum of two years of clinical experience

    •       Experience in care coordination or care management, health coaching, and motivational interviewing

    •       Hands-on experience and comfortability with Electronic Medical Records

    •       Experience using Microsoft Office products such as Teams, Outlook, and Excel

    Competencies Required to Perform the Position and Key Success Factors

    •       Must have advanced computer skills including typing speed, email, internet research, downloading and uploading files, and working in multiple browser windows. Must be extremely comfortable using new technology and software

    •       Skilled in using various computer programs (If you don't love computers, you won't love this position!)

    •       Ability to quickly adapt to new processes, software changes, and priorities

    •       Understand health care goals associated with chronic disease management provided by the practice

    •       Ability to provide prevention and intervention for multiple disease conditions through motivational coaching

    •       Ability to exercise initiative, judgment, organization, problem-solving, and decision-making 

    •       Excellent written and listening skills are a must

    •       Flexible with your availability and interested in working in a telemedicine environment with frequent change and product improvements 

    •       Capable of organizing work to achieve daily/weekly goals and objectives 

    •       Ability to assess, adapt, and calmly respond to changing and crisis environment 

    •       Ability to work collaboratively with both internal and external medical staff and external partners and organizations

    •       Ability to facilitate patient access to community resources

    •       Ability to monitor, assess, and record patient progress against a plan of care

    •       Excellent verbal and written communication skills required

    •       Skilled in care management and patient assessments

    •       Knowledge of care management, resource, and utilization management

    •       A true passion for caring for patients
