Position Summary

Wellbox is a growing healthcare company that provides innovative virtual care solutions for healthcare systems, healthcare payors, employers, and ACOs. These comprehensive solutions help assess and improve the health outcomes of our patients.

We are seeking a Registered Nurse in the state of Michigan to work on a Provider-Delivered Care Management (PDCM) program. The goal of PDCM is to provide patient education, care coordination, and other support services telephonically.  

Essential Functions

·        Conduct recorded phone calls and speak with patients of all ages (or their caregiver if under 18) on their clinical conditions. The content of the phone calls must be medically relevant (appointment reminders and test results do NOT qualify as clinically relevant). The content may include topics such as medication management, skills, health and wellness coaching, self-management training, and education.

·        Build relationships with patients in a non-face-to-face setting

·        Utilize the provider-set care plan or develop a verbal care plan with patients based on their medical diagnosis and partner with the patient to discuss their medical conditions on the phone in collaboration with their medical provider.

·        Set measurable healthcare goals with patients and act as their health coach

·        Document patient visits in Wellbox Operating System (WOS) and in practice’s EHR

·        Report clinical/ escalation issues to the physician and practice in the pre-established workflow set forth by the practice

·        Comply with all HIPAA laws and regulations

·        Call 10-15 patients per hour in instances where no one is answering calls

Competencies Required to Perform the Position

·       Registered Nurse with MICHIGAN license

·       Understanding of health care goals associated with clinical conditions

·       Ability to provide prevention and intervention for multiple clinical conditions

·       Ability to keep patients (and caregivers) engaged and goal-oriented

·       Health coaching and motivational interviewing skills along with a passion for wellness

 Work Environment

This is a remote position. You will need to have high-speed, wired-internet (no satellite, Wi-Fi, or hotspot), and a private space free from noise to work in. For security purposes, all work must be completed on a Wellbox issued computer  

Position Type, Pay, and Expected Hours of Work

This is a remote, independent contractor (1099) position and does not offer benefits. It is estimated that 32-40 hours weekly will be needed to complete the given patient panel. The pay is $1600 biweekly.
