Title: Case Manager
Type: Full-Time
Report to: Director of Compassion Ministries

Work Description: The Case Manager will be responsible for coordinating and overseeing the services provided to neighbors ensuring they receive the necessary support and resources in overcoming barriers to gain stability in areas of their lives such as housing, child care, job training, and placement, healthcare, goal planning, budgeting/financial planning, and any necessary support services to gain physical, financial, and emotional stability.  The Case Manager will serve as an advocate, liaison, and planner, collaborating with neighbors and community organizations to create comprehensive care plans. The Case Manager will also build a support team of volunteers to assist in fulfilling the mission.  

Minimum three (3) years experience in the social/human services field that consists of case management, counseling groups and/or individuals, crisis intervention, and resident advocacy a plus. 

Responsibilities:

  1. Assist in establishing Case Management program standards. 
  2. Conduct initial assessment and screening with neighbors according to program and agency standards to determine eligibility and barriers to self-sufficiency. 
  3. Assist families with SNAP, TANF, Medicaid, and other state assistance programs. 
  4. Develop and implement individualized service plans and individual Plans of Care with neighbors to address identified needs within the established time frame. 
  5. Continuously evaluate neighbor progress utilizing informed interventions to help maintain client engagement and progress in achieving overall well-being and financial self-sufficiency.
  6. Ensure professional service delivery by working to provide timely case management, documentation, and communication with neighbors.
  7. Collaborate and communicate with community partners on resources and services the program cannot provide that are available in the community to households in the program.
  8. Advocate for neighbors' rights’, preferences, and access to other services. 
  9. Assist neighbors in navigating complex systems and overcoming barriers to services. 
  10. Ensure all case notes are properly recorded and reviewed regularly. 
  11. Ensure case volunteers are following established standards. 
  12. Review case volunteers' plans and notes. 
  13. Weekly meetings to discuss Care Plans with the Director of Compassion Ministries. 

Qualifications:

  • Commit to and adhere to 6 Stones’ mission, goals, and core values 
  • Bachelor’s or Master’s degree in social work or related field
  • Minimum three (3) years of work experience in case management or social services
  • Must be bilingual: fluent in Spanish
  • Strong communication, interpersonal, and problem-solving skills 
  • Ability to work effectively in a multidisciplinary team and independently 
  • Knowledge of relevant laws, regulations, and ethical standards 
  • Proficiency in documentation and computer skills 
  • Flexible and adaptable, strong work ethic; demonstrated success working in a fast-paced environment
  • Must have reliable transportation

Work Hours: Monday - Friday 8:00 am - 5:00 pm.  Extended hours when necessary for special events.
