About POTS

Part of the Solution (POTS) offers an array of comprehensive and personalized services to fight poverty and hunger in The Bronx, at no cost to clients. Annually, POTS serves more than 35,000 people, including 12,000 children. Our services are organized around three programmatic pillars: a Dignity and Wellness Program provides shower, mail, haircut and medical and dental services; a Food Security Program offers hot meals and groceries; and a Long-Term Stability Program provides guidance on applications for public benefits and mentoring for long-term personal growth.

About the position

The Case Manager will assist clients in obtaining public benefits; accessing detoxification, psychiatric and domestic violence prevention services; obtaining temporary and permanent housing; and performing holistic case management services that help families move from crisis to stability and, ultimately, self-sufficiency according to POTS Theory of Change.

 

Responsibilities

Commitment to POTS: Belief in the programs and mission of the organization. Motivated to achieve a desired goal and ability to plan, manage time effectively and work towards it proactively, going the extra mile if necessary. Will be comfortable being accountable for ambitious goals and understands the role of measurable data in achieving results.

Benefits applications: Assist clients in obtaining and maintaining cash and non-cash benefits such as Supplemental Nutritional Assistance Program (SNAP), WIC, Medicaid, SSI/SSD, SCRIE, DRIE, and COVID-19 recovery benefits. Help clients navigate access to various forms of temporary and permanent housing, including Section 8 re-certifications, and low-income housing applications.

Referral services: Provide access to internal and external services including emergency food, haircuts, a safe and secure mailbox, job training and placement programs, ESL, basic education and GED programs, detoxification centers, shelters, free income tax preparation, health care navigators, medical and psychiatric care centers, domestic violence prevention and assessment resources and other services that promote greater stability and independence.

Client stabilization and case planning: For clients who seek more in depth services, the case manager will perform holistic case management services for individual or family needs including using a strong referral network to meet those needs.

Client data management: Record and document interactions with clients. Follow-up with referral agencies to confirm that clients received the services they need. Keep client data safe and secure.

 

Required Skills

  • Proficient in Microsoft Office and database management software
  • Fluent in Spanish
  • Bachelor’s Degree
  • Willingness to travel off-site
  • Excellent customer service skills
  • Ability to work independently
  • Cultural competence and sensitivity to relate professionally with clients and their families
  • Candidate must be able to work on Saturdays. 

 

Preferred

  • Knowledge of public benefits access in New York though work or internship experience
  • Case management experience
