The Post Adoption/Post Permanency Case Manager is a member of the Permanency Support Services program. The Post Adoption/Post Permanency Case Manager will work with the Supervisor of the Permanency Support program to provide information and referral services to post adoption or post permanency families seeking services, as needed. The Post Adoption/Post Permanency Case Manager will develop and provide outreach to the community to increase awareness of services available to families. The Post Adoption/ Post Permanency Case Manager will work to maintain an effective working relationship between all Arms Wide programs and other Child Placing Agencies in order to provide a continuum of service to families granted Permanent Managing Conservatorship (PMC) of children from foster care and/or Post Adoption families. Arms Wide is looking for someone who is:

Experience/Skills Required
- Carries a caseload of post adoption clients/post permanency clients.
- Coordinates family support events - Frequency determined based on funding availability.
- Conducts outreach services as needed
- Assists with information and referral for TDFPS contracts, including services to families out of state that are eligible for post adoption or post permanency services through the contracts.
- Documents and determines client eligibility, maintains case records, assesses third party resources and ensures these resources are applied when available.
Experience/Skills Preferred
- Basic knowledge of human growth and development
- Understanding of family dynamics theory and practice
- Interviewing skills
- Ability to evaluate parenting skills
- Ability to work independently and as a team member
- Ability to use information from other disciplines in evaluating children
Travel Required
- Reliable transportation required. Mileage reimbursed.
