The Permanency Support Services Case Manager will provide a continuum of services to families granted Permanent Managing Conservatorship (PMC) of children and/or those who have adopted (Post Adoption) children from the foster care system. The Case Manager will offer information and referral services to post-adoption and post-permanency families seeking support. Additionally, the Case Manager will conduct outreach activities to increase awareness of available services.
Permanency Support Services aims to strengthen family functioning, enhance the quality of life for family members, promote safety and stability within the home, support families in coping with the children’s history of abuse and/or neglect, and prevent permanent or long-term removal of children from the family. The ultimate goal of the Case Manager is to preserve and strengthen the family unit.

Experience/Skills Required
- Bachelor’s degree in social work or related area, with, at least, one year of child welfare or child placing experience.
- Bilingual (Spanish/English)
- Requires good, basic, clerical and administrative skills with emphasis on verbal and written communication skills; positive attitude; willingness to take initiative; and ability to maintain confidentiality.
- Basic knowledge of human growth and development
- Understanding of family dynamics theory and practice
Experience/Skills Preferred
- Master’s degree in social work or Behavioral Science field preferred
- Background in trauma-informed practices
- Experience providing trauma informed care focused case management, specifically, Trust-Based Relational Intervention (TBRI)
- Ability to work independently and as a team member
Travel Required
- Reliable transportation required. Mileage reimbursed.
