The Post Adoption Outreach Case Manager will work with the Supervisor of the Post Adoption/Post Permanency services program to provide information and referral services to Post Adoption families seeking services, as needed.  The Post Adoption/Outreach Case Manager will develop and provide outreach to the community to increase awareness of services available to families.  The Post Adoption/ Outreach Case Manager will work to maintain an effective working relationship between all Arms Wide programs and other Child Placing Agencies in order to provide a continuum of service to Post Adoption families. Arms Wide Permanency Support Services seeks to strengthen family functioning, and to increase the quality of life for all family members. Case management consists of the planning and coordination of permanency services for an adoptive family and their adoptive child. The goal is support of the family relationships, strengthen family supports to assist with promoting safety and security within the family, cope with any history of abuse in the child’s background, and avoid permanent or long-term removal of an adopted child(ren) from the family.  The case manager’s goal is to help the family remain intact.

Experience/Skills Required:

  • Manage a caseload of post adoption clients.
  • Conduct outreach services regarding post adoption.  Outreach service goals to be developed monthly with in-person site visits a required and valued part of this role.
  • Public informational presentations in such venues as: family court, resource fairs, churches, schools, etc.
  • Provide information and referral for TDFPS contracts, including services to families out of state that are eligible for post adoption or post permanency services through the contracts. This includes responding to inquiry calls under a dedicated assignment rotation schedule.
  • Document and determine client eligibility, maintain case records, assess third party resources and ensure these resources are applied when available.
  • Bachelor’s Degree in Social Work, or related area, with two years of child welfare or child placing experience.

Experience/Skills Preferred:

  • Community engagement/outreach experience preferred
  • Bilingual in Spanish/English preferred
  • Experience in child welfare preferred
  • Case management skills preferred

Travel Required:

  • Reliable transportation required. Mileage reimbursed.
