The Manager of Integrated Care Management will collaborate with the CMO and Director of Population Health to develop, stand-up, and iteratively improve a highly effective care management program that provides population-specific support based on actionable clinical data from the member’s provider care team.  The program will optimally and appropriately anticipate member needs in a manner that reduces unnecessary utilization of services and support members with the most impactable circumstances. The Manager will cross-cover for dedicated care manager staff and provide care management services for certain complex, high risk plan members.

The successful incumbent will exercise agility in the execution of plans and consistently role-model diplomacy and adaptability in fostering relationships of trust amongst diverse stakeholders that create a spirit of collaboration, good will, and partnership.

EDUCATION & EXPERIENCE

·          3+ years in care management leadership roles within a clinically integrated network and/or in a health plan or similar setting

·          3+ years of direct experience in providing disease management, transition of care/discharge planning, and complex care management services

·          Clinical degree (RN, LPN, or related degree) required

·          Advanced education (MBA, MHA or related degree) preferred
